PTSD is characterized by maladaptive rather than adaptive coping mechanisms. While the triggering event is indeed outside the range of usual human experience, the diagnostic criteria focus on the failure to recover and the subsequent psychological distress rather than a successful or healthy adaptation to the initial traumatic stressor.

Individuals with PTSD often exhibit emotional numbing or a diminished interest in significant activities, rather than talking openly. Many experience difficulty disclosing the trauma due to shame, guilt, or the desire to avoid the intense autonomic arousal and re-experiencing symptoms that occur when discussing the event.

Patients typically demonstrate avoidance behavior rather than seeking out the location of the trauma. Purposefully visiting the site of the trauma is often a component of controlled exposure therapy led by a clinician, but it is not a symptomatic feature of the disorder itself in untreated individuals.

Allowing a depressed patient to remain isolated increases the risk of rumination and self-harm. Loneliness exacerbates feelings of worthlessness and hopelessness, which are core symptoms of major depressive disorder. Therapeutic engagement and social interaction are preferred over prolonged periods of solitary meditation in an acute clinical setting.

While reducing self-absorption is a goal, it is not the highest clinical priority. Distraction techniques may provide temporary relief from depressive cognitions, but they do not address the underlying safety concerns or the neurochemical imbalances associated with severe depression and potential suicidal ideation in the patient.

Assigning a leadership role to a patient feeling worthless can be counterproductive and overwhelming. The patient may lack the cognitive energy or executive function to manage responsibilities, and a perceived failure in this role could significantly deepen their existing sense of inadequacy and psychological distress.

This response utilizes logic to challenge a delusion, which is generally ineffective in paranoid schizophrenia. This statement provides false reassurance and fails to address the specific sensory perception of the voices. In a psychiatric context, positive symptoms do not refer to the patient's emotional outlook or optimism. Using the term positive in this manner is a colloquial misunderstanding of clinical terminology, which specifically categorizes the manifestations of psychosis based on the presence or absence of normal functions.

Anosognosia, or the lack of insight into one's illness, is common in schizophrenia but is not the definition of a positive symptom. Being positive that a diagnosis is wrong is a behavioral manifestation of poor insight or denial rather than an additive psychotic symptom like a hallucination.

These are actually classified as negative symptoms. Negative symptoms involve a deficit or loss of normal functions, such as avolition (lack of motivation), alogia (reduced speech), and anhedonia (loss of interest). They represent a decrease in the patient's baseline functioning rather than an added experience.

Covert anger might result in a nurse being overly rigid or punitive rather than struggling to set limits. While suppressed resentment can interfere with the therapeutic relationship, it is typically characterized by passive-aggressive behaviors rather than the inability to maintain the necessary structure and clinical rules.

Value dissonance involves a conflict between the nurse's personal beliefs and the patient's lifestyle or choices. While this can cause moral distress, it usually manifests as judgment or avoidance rather than a specific clinical inability to provide the structure and limit-setting required for behavioral management.

Empathy is a core therapeutic tool and should not prevent a nurse from setting limits. True clinical empathy involves understanding the patient's perspective while remaining objective. It is the loss of this objectivity, rather than the presence of empathy itself, that leads to boundary issues.

Wishful thinking is a cognitive bias characterized by the formation of beliefs based on what might be pleasing to imagine rather than on evidence or rationality. Unlike cognitive reframing, which is a structured therapeutic skill used to improve mental health, wishful thinking is often a defense mechanism that ignores reality.

Guided imagery is a relaxation technique that involves focusing on peaceful, calming mental images to reduce stress or pain. While it is a therapeutic intervention, it focuses on sensory visualization and autonomic nervous system regulation rather than the active analytical process of examining and restructuring negative thought patterns.

Confrontational assertion is a communication style where an individual directly and sometimes aggressively addresses a conflict or behavior. It does not involve the internal psychological process of examining one's own thoughts or restating them, but rather focuses on external interpersonal interactions and setting boundaries or demands.

Encouraging a patient to imagine calm circumstances refers to visualization or guided imagery. While these are effective stress management tools, they address the physiological symptoms of stress rather than the cognitive "ways of thinking" mentioned by the doctor, which require a direct focus on thought patterns.

Journaling is a helpful adjunct therapy that can facilitate self-reflection, but simply providing a journal does not inherently teach the skill of restructuring negative thoughts. Without specific instruction on cognitive techniques, the patient might simply record their negative thoughts without learning how to change or challenge them.

Using instruments for feedback about bodily functions refers to biofeedback. This technique helps patients gain voluntary control over autonomic functions like heart rate and muscle tension. Like guided imagery, it addresses the physical manifestations of stress rather than the cognitive distortions that trigger the stress response.

This response is overly blunt and may be perceived as aggressive or defensive. It risks damaging the therapeutic alliance by creating a hostile environment. While it sets a boundary, it does so without providing a therapeutic rationale, which can make the patient feel rejected or judged.

Asking "why" is often non-therapeutic in psychiatric nursing as it can put the patient on the defensive and demand an explanation for a behavior they may not consciously understand. It shifts the focus to the patient's motives in a way that feels like an interrogation.

This statement is authoritarian and focuses on the power dynamic of the relationship. It emphasizes the nurse's control rather than the therapeutic purpose of the interaction. It may discourage the patient from participating actively in their own care or sharing information openly.

Moderate anxiety typically involves a narrowed perceptual field where the individual hears, sees, and grasps less information than normal. However, the person remains capable of problem-solving with assistance and does not lose touch with reality or exhibit the severe motor incoordination seen in panic.

"Manic anxiety" is not a standard clinical stage in the hierarchy of anxiety levels (mild, moderate, severe, panic). While mania in bipolar disorder involves impulsivity and agitation, the specific description of losing touch with reality due to overwhelming anxiety fits the clinical definition of the panic stage.

Severe anxiety involves a significantly reduced perceptual field; the person may focus on one detail or many scattered details. While the individual experiences great discomfort and physical symptoms (like hyperventilation), they generally maintain a connection to reality, unlike the "loss of touch" described in panic.
