Introduction

Classification of Anticoagulant medications

-Anticoagulants can be classified into four main groups:
- heparins,
- vitamin K antagonists,
- direct oral anticoagulants (DOACs), and
- direct thrombin inhibitors (DTIs)
Heparins
- Heparins are polysaccharides derived from animal sources that bind to antithrombin III (ATIII), a natural inhibitor of thrombin and factor Xa
- Heparins can be unfractionated heparin (UFH) or low molecular weight heparin (LMWH)
- UFH has a variable molecular weight and a short half-life of 1 to 2 hours. It is administered intravenously (IV) or subcutaneously (SC) and requires frequent monitoring of activated partial thromboplastin time (aPTT)
- LMWH has a lower molecular weight and a longer half-life of 4 to 6 hours. It is administered SC and does not require routine monitoring of aPTT. It has a more predictable anticoagulant effect and fewer adverse effects than UFH
- The main adverse effect of heparins is bleeding. Other adverse effects include hypersensitivity reactions, osteoporosis, alopecia, and heparin-induced thrombocytopenia (HIT)
- HIT is a serious complication characterized by a paradoxical drop in platelet count due to antibody-mediated platelet activation and aggregation. It can lead to arterial or venous thrombosis and requires immediate discontinuation of heparin
- The antidote for heparin overdose is protamine sulfate, which neutralizes heparin by forming a complex with it. Protamine should be given slowly IV within 30 minutes of heparin administration
- The dosage of heparin is based on body weight and adjusted according to the target therapeutic range of aPTT. The normal aPTT is 40 seconds, and the therapeutic range is 1.5 to 2 times normal, or between 60 to 80 seconds

- The most commonly used vitamin K antagonist is warfarin (Coumadin)
- Warfarin is indicated for the prevention and treatment of thromboembolic disorders such as DVT, PE, AF, stroke, MI, and mechanical heart valves
- Warfarin has a delayed onset of action of 2 to 5 days and a long half-life of 20 to 60 hours. It is administered orally and requires frequent monitoring of prothrombin time (PT) and international normalized ratio (INR)
- The main adverse effect of warfarin is bleeding. Other adverse effects include skin necrosis, purple toe syndrome, alopecia, and fetal harm
- The antidote for warfarin overdose is vitamin K (phytonadione), which reverses the effect of warfarin by restoring the synthesis of clotting factors. Vitamin K can be given orally or IV depending on the severity of bleeding. IV vitamin K should be given slowly over 30 minutes to avoid anaphylaxis
- The dosage of warfarin is based on body weight and adjusted according to the target therapeutic range of INR. The normal INR is 1.0, and the therapeutic range depends on the indication. For most indications, the therapeutic range is 2.0 to 3.0. For mechanical heart valves or recurrent systemic embolism, the therapeutic range is 2.5 to 3.5

- DOACs include factor Xa inhibitors (e.g., rivaroxaban, apixaban, edoxaban) and direct thrombin inhibitors (e.g., dabigatran)
- DOACs are indicated for the prevention and treatment of thromboembolic disorders such as DVT, PE, AF, stroke, and MI
- DOACs have a rapid onset of action of 2 to 4 hours and a short half-life of 8 to 15 hours. They are administered orally and do not require routine monitoring of PT or INR
- The main adverse effect of DOACs is bleeding. Other adverse effects include gastrointestinal disturbances, rash, and hypersensitivity reactions
- The antidotes for DOAC overdose are specific reversal agents that bind to the drug and prevent its activity. For factor Xa inhibitors, the antidote is andexanet alfa (Andexxa). For dabigatran, the antidote is idarucizumab (Praxbind)
