Replacing the securing mechanism is one of the final steps of tracheostomy care and should not be performed prematurely. Doing this prior to cleaning increases the risk of accidental decannulation if the tube is not properly stabilized during the entire process. Security is a priority throughout the procedure.

Cleaning the peristomal area is a core component of the procedure itself, not a preparatory action. Performing this before the setup and communication plan violates the sterile sequence and logical flow of nursing interventions. It follows the assessment and preparation of the patient.

Assessing the tension of the ties is part of the ongoing assessment and final stabilization, but it does not take precedence over establishing emergency communication. While ensuring the knot is secure is important, the nurse must first ensure the patient can signal for help. Assessment of ties occurs after the new ties are applied.

Using oil-based lubricants like petroleum jelly is contraindicated because it poses a significant risk for lipid pneumonia if aspirated into the lungs. Only water-soluble lubricants should be utilized to facilitate the passage of the catheter through the nasal turbinates. Petroleum products can also damage the catheter material.

Suction must never be applied during the insertion phase as it causes mucosal damage and depletes the patient's oxygen reserves prematurely. The catheter should be advanced carefully without suction until the carina or a cough reflex is reached. Suction is only activated during the withdrawal phase.

While pre-oxygenation is essential, doing so for 30 minutes is clinically excessive and can lead to oxygen toxicity or absorption atelectasis. Standard protocols recommend hyperoxygenation with 100% oxygen for only 30 to 60 seconds immediately prior to and after the procedure. Prolonged exposure is unnecessary and potentially harmful.

This statement describes the technique for peak flow meters, not incentive spirometry. Exhaling forcefully into the device is a misapplication of the tool and does not contribute to the goal of lung expansion. It may lead to dizziness or unnecessary physical exertion without clinical benefit.

Using the device only three times a day is clinically insufficient for a postoperative or immobilized patient. Standard protocols typically mandate using the spirometer 5 to 10 times every hour while awake. Infrequent use allows for the progression of atelectasis and stasis.

Proper hygiene for the device focuses on the mouthpiece, which should be cleaned after each use. While the device should be kept clean, the weekly washing of the entire device is not the priority of patient education regarding its therapeutic function. Clinical effectiveness is the primary teaching focus.

While returning the client to bed is eventually necessary, it is a secondary action that does not address the immediate threat to the airway. Allowing the client to walk with an open pleural space increases the risk of rapid respiratory decompensation. Reconnection must happen at the site of the incident.

Auscultation of lung sounds is an important assessment, but it should be performed after the system has been secured. Delaying the repair of the drainage system to perform an assessment allows more air to enter the pleural space, potentially worsening the client's condition. Intervention takes priority over assessment in this emergency.

Forcing the client to cough while the tube is disconnected can exacerbate the loss of negative pressure and increase the risk of air entering the chest. While coughing helps expel air in a functional system, it is dangerous when the system is open to the atmosphere. It is an inappropriate and harmful instruction in this context.

The logical sequence of pulmonary hygiene must begin with positioning to allow gravity to start the drainage process before applying percussion. Suctioning or coughing should be the final step to remove the secretions that have been mobilized to the upper airways. The suggested order is mechanically inefficient.

Performing these exercises immediately after breakfast significantly increases the risk of aspiration due to the Trendelenburg or side-lying positions required. The mechanical jarring of percussion on a full stomach can induce nausea and vomiting. Timing should always favor an empty stomach for comfort and safety.

Hyperoxygenation is typically reserved for invasive procedures like tracheal suctioning and is not a standard requirement for chest physiotherapy. While monitoring oxygen saturation is important, a 3-minute delivery of 100% oxygen is an excessive and non-standard intervention for this procedure. It is not supported by routine clinical protocols.
