Separating these exercises based on meal times is clinically inefficient and lacks physiological justification for preventing complications like atelectasis. Respiratory interventions must be frequent and consistent rather than scheduled around gastric emptying or nutritional intake. This timing would allow for prolonged periods of lung stasis.

Vigorous, uncontrolled coughing can cause excessive pain, increase thoracic pressure, and lead to the collapse of small airways. It places unnecessary stress on surgical sites and may cause the patient to avoid further respiratory exercises due to discomfort. Controlled techniques are always preferred over forceful efforts.

While these techniques improve ventilation and oxygenation, the frequency of four times a day is insufficient for preventing postoperative pneumonia. Postoperative protocols require more frequent intervention, typically every 1 to 2 hours, to maintain lung expansion and prevent the accumulation of stagnant secretions. The dosage is inadequate for high-risk patients.

Unlike beta-2 agonists, corticosteroids do not exert a direct effect on the smooth muscle to cause immediate relaxation or expansion. While they facilitate better airflow over time by reducing swelling, they are not classified as bronchodilators in a pharmacological sense. They do not provide rapid rescue relief.

Steroids do not act as expectorants or mucolytics to directly assist the mechanical act of coughing. While a decrease in inflammation may eventually lead to less mucus production, the drug's mechanism is not designed to stimulate the cough reflex or clear secretions. It is an anti-inflammatory, not a mechanical aid.

Corticosteroids are actually immunosuppressive and can potentially mask or increase the risk of developing a secondary infection. They do not have antimicrobial properties and are administered alongside antibiotics if an infection is present. Their goal is to manage the inflammatory response, not kill pathogens.

Crackles are discontinuous, popping sounds heard primarily during inspiration when collapsed alveoli or small airways snap open. They are often associated with fluid in the air spaces or interstitial edema, rather than a musical or continuous vibration. They are often described as sounding like Velcro or hair rubbing together.

Bronchial sounds are normal, loud, high-pitched sounds heard specifically over the trachea and mainstem bronchi. While they are loud, they are not musical or adventitious and represent the normal turbulence of air in large conduits. Finding them in peripheral lung fields would be abnormal, but the description does not fit.

Pleural friction rub is a dry, creaking, or grating sound caused by the rubbing together of inflamed pleural surfaces during the respiratory cycle. It is not musical in nature and is often compared to the sound of two pieces of leather rubbing together. It usually ceases when the patient holds their breath.

Pleural effusion refers to the abnormal accumulation of fluid, such as transudate or exudate, within the pleural space rather than air. While it also compresses the lung tissue and impairs ventilation, the physical state of the matter is liquid. It is often secondary to heart failure or malignancy.

Hemothorax describes the presence of blood in the pleural space, typically following thoracic trauma or vascular rupture. It is a distinct clinical entity that requires drainage to prevent clot formation and fibrothorax. The density of blood is significantly different from the air described in the stem.

Atelectasis is a condition of alveolar collapse within the lung parenchyma itself, not the accumulation of a substance in the pleural space. It occurs when the small air sacs become deflated or filled with alveolar fluid. It is an internal lung tissue problem rather than a pleural space issue.

Pulse oximetry specifically measures arterial oxygen saturation by detecting the pulsatile flow of blood in the capillaries. It does not measure venous blood, which has significantly lower oxygen levels after oxygen has been unloaded at the tissue level. The “pulse” in the name refers to the arterial beat.

This is a non-invasive, external procedure that does not require any pre-test restrictions or dietary modifications. It is a bedside tool used for immediate assessment and does not involve the laboratory processing or blood draws associated with fasting requirements. It can be performed at any time.

While useful for monitoring, oximetry cannot measure pH, partial pressure of carbon dioxide (pCO2), or bicarbonate levels. Therefore, it is a supplement to, but not a replacement for, arterial blood gas analysis when a complete acid-base assessment is required. It provides a limited picture of respiratory status.
