It demonstrates proper neuraxial distribution of the local anesthetic within the epidural space. This predictable sensory block confirms correct catheter positioning rather than vascular cannulation.

A mild blood pressure decline after 15 minutes reflects normal sympathetic blockade from regional anesthesia. Vasodilation occurs gradually as the local anesthetic diffuses across neural segments. Sympathectomy causes peripheral pooling, which is managed with fluids rather than indicating vascular entrance.

Feeling pressure during contractions is normal because epidural analgesia primarily blocks pain fibers, not tactile perception. Sensory fibers carrying visceral pressure remain partially unblocked during successful neuraxial analgesia. This finding confirms functional analgesia and rules out systemic intravascular administration.

Positioning the client in a side-lying or sitting posture optimizes vertebral exposure for catheter insertion. A Bromage score of 1 represents a partial motor block where the client can move their feet and flex their knees. However, the client is unable to perform a straight leg raise against gravity. This score reflects partial recovery or mild motor blockade during epidural infusion management.

A Bromage score of 2 represents an almost complete motor block where the client can move their feet only. The client is completely unable to flex their knees or lift their lower extremities. This indicates significant blockage of motor pathways with minimal distal movement remaining in the toes.

Preparing the client for placement without communicating the low platelet count bypasses critical patient safety protocols. Inserting an epidural needle with impaired primary hemostasis risks non-compressible intraspinal bleeding. The nurse must withhold preparation until the anesthesia provider evaluates hematologic status.

Administering an intravenous fluid bolus is indicated to prevent post-block hypotension, but it cannot correct underlying platelet dysfunction. Furthermore, expanding plasma volume further dilutes circulating platelets, exacerbating dilutional thrombocytopenia. Intravenous fluid boluses do not diminish bleeding risks.

Reassuring the client that platelets do not impact safety provides false information and ignores severe procedural contraindications.
