Using a humidifier might increase comfort by keeping the airway moist, especially if the client uses a continuous positive airway pressure machine, but it does not address the underlying pathophysiology of obstructive sleep apnea. The primary issue in this condition is the physical collapse of the upper airway during sleep. Adding moisture to the air does not provide the structural support or the necessary pressure to keep the pharyngeal tissues from obstructing the flow.

Taking a sleeping pill is contraindicated for clients with obstructive sleep apnea. Sedatives and hypnotics relax the muscles of the upper airway even further, which can worsen the frequency and severity of the apneic episodes. These medications also blunt the body's natural arousal response to hypoxia and hypercapnia, preventing the client from waking up briefly to resume breathing. This can lead to dangerous drops in oxygen saturation levels during the night.

Sleeping on the back, or the supine position, is generally discouraged for individuals with obstructive sleep apnea. In this position, gravity causes the tongue and the soft palate to fall backward against the posterior pharyngeal wall, increasing the likelihood of airway obstruction. Most patients are encouraged to use positional therapy, such as sleeping on their side, to help keep the airway open by preventing the tongue from occluding the throat.

Nausea is not a classic manifestation of hypoxemia during an acute asthma attack. While a patient might feel generally unwell or experience gastrointestinal upset due to the stress of the event or the side effects of medications like bronchodilators, it is not a direct indicator of low oxygen levels. The clinical focus during an asthma assessment is on respiratory rate, effort, breath sounds, and neurologic status to determine the severity of the hypoxic state.

Hypotension is typically a late sign of severe respiratory failure or shock rather than an early manifestation of hypoxemia in an asthma attack. Initially, the body compensates for hypoxia by increasing the heart rate and systemic blood pressure through sympathetic nervous system activation. A significant drop in blood pressure would indicate that the body's compensatory mechanisms are failing or that there is severe tension pneumothorax or cardiovascular collapse occurring as a complication.

Dysphagia, or difficulty swallowing, is not an expected finding specifically linked to hypoxemia. While the patient may have difficulty speaking or swallowing due to the rapid breathing and the use of accessory muscles, dysphagia itself is usually related to neuromuscular disorders or physical obstructions in the esophagus or throat. In an asthma attack, the primary issue is the narrowing of the lower airways, which affects ventilation rather than the physical act of swallowing.

Performing up to four passes is generally avoided because each suctioning event increases the risk of mucosal trauma, hypoxia, and cardiac arrhythmias. The standard nursing practice is to limit suctioning to a maximum of two or three passes per session. Frequent passes can deplete the functional residual capacity of the lungs and cause significant distress to the client. The nurse must allow for adequate recovery and reoxygenation between each pass to maintain physiological stability.

Limiting each suction pass to 25 seconds is dangerously long and can lead to severe hypoxemia and vagal stimulation. The evidence-based limit for applying suction is typically 10 to 15 seconds. Prolonged suctioning removes not only secretions but also the air the client needs for oxygenation. Monitoring the heart rate and oxygen saturation during the procedure is vital, and the nurse should immediately stop if the client shows signs of bradycardia or a significant drop in saturation.

Applying suction while advancing the catheter into the trachea is incorrect because it causes unnecessary trauma to the tracheal mucosa and increases the risk of hypoxia. The catheter should be inserted without suction until resistance is met or the client coughs. Suction should only be applied intermittently or continuously while withdrawing the catheter using a rotating motion. This technique ensures that secretions are removed effectively while minimizing damage to the delicate tissues lining the client's airway.

Oxygen saturation is measured using pulse oximetry, which is a non-invasive method using light waves, or through an arterial blood gas analysis. While oxygen levels are important in respiratory care, they are not the primary measurement of a pulmonary function test. A PFT focuses on volumes and capacities of the lungs rather than the percentage of hemoglobin saturated with oxygen. Confusing these two could lead the patient to misunderstand the purpose of the more complex breathing maneuvers required.

Measuring the balance of oxygen, carbon dioxide, and pH is the purpose of an arterial blood gas test, not a pulmonary function test. An ABG provides information about gas exchange at the alveolar-capillary level and the body's acid-base status. Normal pH ranges from 7.35 to 7.45. While these values are crucial for patients with emphysema who may retain carbon dioxide, a PFT is a different diagnostic tool that assesses the physical mechanics and airflow limits of the lungs.

The movement of oxygen and carbon dioxide between the lungs and the blood refers to diffusion capacity, which can be part of a comprehensive PFT but is not the general definition. This specific process is often measured by the DLCO test. However, the most fundamental description of a PFT for a patient involves the general movement of air. Emphysema specifically involves the destruction of alveoli, which reduces the surface area for this exchange and causes air trapping.

Respiratory alkalosis is not expected in a client with stable COPD and emphysema. Instead, these clients are prone to respiratory acidosis due to chronic carbon dioxide retention. As alveolar walls are destroyed, the lungs lose their elastic recoil, leading to air trapping and impaired gas exchange. This results in an elevated partial pressure of carbon dioxide and a lower pH. Normal arterial pH is 7.35 to 7.45, and a PCO2 above 45 mmHg indicates CO2 retention.

An oxygen saturation level of 96 percent is generally considered normal for a healthy individual but is often higher than what is seen in a client with significant COPD. Clients with emphysema typically have lower baseline saturations, often ranging from 88 percent to 92 percent. Their drive to breathe may become dependent on low oxygen levels rather than high carbon dioxide levels. Expecting a high normal saturation would be unrealistic for someone presenting with shortness of breath and emphysema.

Petechiae, which are small red or purple spots caused by minor hemorrhage into the skin, are not a manifestation of COPD or emphysema. Petechiae are more commonly associated with platelet disorders, certain infections, or trauma. In the context of respiratory assessment, a nurse would be more likely to look for cyanosis, which is a bluish discoloration of the skin or mucous membranes indicating poor oxygenation, rather than petechiae, which have no direct scientific link to alveolar destruction.
