Obtain a STAT electrocardiogram (EKG).: An EKG evaluates cardiac rhythm and ongoing ischemic changes but does not assess neurological status. New disorientation during alteplase infusion raises concern for intracranial hemorrhage. Cardiac monitoring is important, but it does not address the immediate neurologic emergency suggested by acute mental status changes.

Prepare the client for percutaneous cardiac intervention (PCI).: PCI is considered if thrombolytic therapy fails or if there is persistent ischemia. Disorientation is not an indication of failed reperfusion but rather a potential bleeding complication. Preparing for PCI would delay evaluation of a potentially life-threatening neurologic event.

Obtain STAT cardiac enzymes.: Cardiac enzymes help evaluate myocardial injury but are not useful in assessing acute neurologic deterioration. Elevated enzymes confirm myocardial infarction but do not explain sudden changes in mental status. The priority is evaluating for a hemorrhagic complication related to thrombolytic therapy.

Increased respiratory rate: An increased respiratory rate may indicate persistent heart failure, hypoxia, or worsening pulmonary congestion rather than improvement. Dobutamine is intended to enhance myocardial contractility and improve cardiac output, which should reduce compensatory tachypnea associated with poor perfusion.

Decreased blood pressure: Dobutamine is a beta-1 adrenergic agonist that increases myocardial contractility and stroke volume, typically improving cardiac output and supporting blood pressure. A decrease in blood pressure may indicate worsening cardiac function or excessive vasodilation, not effective therapy.

Decreased blood glucose: Dobutamine does not directly lower blood glucose levels. Its primary action is to stimulate beta-1 receptors in the heart to increase contractility and cardiac output. Blood glucose changes are not used as indicators of therapeutic effectiveness for this medication.

Muted systolic murmur: Mitral valve stenosis typically produces a diastolic murmur, not a systolic one. A muted systolic murmur does not specifically indicate progression of mitral stenosis.

Upper extremity weakness: Weakness of the upper extremities is not a typical manifestation of mitral valve stenosis progression. It may suggest a neurologic or musculoskeletal issue instead.

Oxygen saturation of 93%: Although slightly decreased, an oxygen saturation of 93% is only mildly low and not specific for progression of mitral stenosis. Dyspnea related to pulmonary congestion is a more significant and classic indicator.

Sinus bradycardia with a rate of 56 on the monitor: A heart rate of 56 bpm may be normal for some clients, especially those on beta-blockers. While it should be documented and monitored, it is not the priority concern prior to the procedure.

Client reports chest pain that occurred yesterday: Chest pain history is relevant but expected in clients undergoing coronary arteriography. Pain that occurred yesterday and is not current is less urgent than the metformin issue.

Blood glucose level 144 mg/dL: A glucose level of 144 mg/dL is mildly elevated but not critical in the context of this procedure. It does not pose the same immediate risk as contrast administration with recent metformin use.

Would you like to speak with a chaplain prior to surgery: While spiritual support may be helpful, this option changes the focus away from exploring the client’s expressed fear. The nurse should first assess the client’s concerns before offering additional resources.

What support systems do you have to assist you: This question may be appropriate later, but it does not directly address the client’s immediate fear of dying. The priority is to explore the expressed anxiety.

This is a routine surgery and the risk of death is very low: Providing false reassurance dismisses the client’s feelings and may undermine trust. Emergent aortic aneurysm repair is a high-risk surgery, so minimizing the risk is inappropriate and nontherapeutic.

EKG showing sinus tachycardia, rate 112: After a heart transplant, the heart is denervated, and mild resting tachycardia (90–110 bpm) is common. A rate of 112 can occur and, by itself, does not specifically indicate rejection.

Blood pressure 154/90: Mild hypertension is common in transplant recipients due to immunosuppressive therapy (such as corticosteroids or calcineurin inhibitors). This finding alone does not strongly suggest rejection.

White blood cell count 11,000: A WBC count of 11,000 is only slightly elevated and may reflect stress or mild infection. It is not a specific indicator of transplant rejection.

Mitral murmur: A mitral murmur is associated with mitral valve disorders such as mitral stenosis or mitral regurgitation, not inflammation of the pericardium.

Pleural friction rub: A pleural friction rub is heard with pleuritis and is related to lung and pleural inflammation. It is respiratory in origin and changes with breathing, unlike a pericardial friction rub.

S3 or S4: An S3 is commonly associated with heart failure, and an S4 is often linked to decreased ventricular compliance, such as in hypertension. These are not characteristic findings of acute pericarditis.

Prepare for transcutaneous pacing: Transcutaneous pacing is used to treat symptomatic bradycardia or heart block, which are not primary concerns in hypertrophic cardiomyopathy. HCM typically involves obstruction of ventricular outflow and risk for arrhythmias, but pacing is not a first-line intervention.

Instructing the client to start an aerobic exercise plan: Aerobic exercise may increase the risk of sudden cardiac death in clients with HCM due to potential arrhythmias and increased myocardial oxygen demand. Exercise recommendations are usually individualized and often limited rather than broadly prescribed.

Instruct the client to perform a Valsalva maneuver: The Valsalva maneuver transiently decreases venous return and can exacerbate left ventricular outflow obstruction in HCM, potentially worsening symptoms. It is generally contraindicated rather than recommended.
