Give the medication early for any pain score greater than 7: Administering a narcotic before the prescribed PRN interval violates safe medication administration principles and provider orders. Doing so increases the risk of opioid toxicity, respiratory depression, and oversedation, especially with drugs such as morphine. Medication timing must follow the ordered frequency unless a new order is obtained.

Call the prescriber and request another option: While contacting the prescriber may ultimately be necessary, the nurse must first conduct a comprehensive pain assessment. Clinical decision-making should be based on objective findings, pain characteristics, previous response to medication, and assessment of adverse effects before escalating to the provider.

Discuss the situation with Mr. Brown, and tell him you will advocate for alternative pain management when the physician does rounds for the day: Delaying intervention until physician rounds does not adequately address acute severe pain. Pain management is a priority, and timely assessment and communication are necessary rather than postponing action for several hours.

Double checking that the client is not allergic to the medication ordered: Assessing allergies is a critical safety step before administration, but it is part of safe medication administration rather than the definition of order verification. Verification specifically focuses on confirming the accuracy and correctness of the written or electronic order before implementation.

Clarifying the order with the charge nurse: If an order is unclear, incomplete, or potentially unsafe, it must be clarified with the prescribing provider—not the charge nurse. Consulting a charge nurse may provide guidance, but it does not constitute formal verification of the medication order.

Ensuring that the medication ordered has been received by pharmacy: Pharmacy receipt and dispensing relate to medication processing and distribution. Verification, however, occurs prior to or during transcription and ensures that the provider’s order is accurate, complete, and correctly carried forward into the medication administration system.

Explain to Mr. Brown that his physician has ordered the medication and it is best for him to take it: Simply reinforcing the provider’s authority does not address the patient’s concern and may override an important safety cue. Patients often recognize discrepancies in their usual regimen, and dismissing their concern could result in administering an incorrect medication.

Check the sign above the bed to the MAR & check the armband: The sign above the bed should never be used as a primary identifier because room assignments can change. While checking the armband is essential, including the bed sign introduces risk and does not align with best-practice identification policies.

Call the client by name on MAR and observe his response: Calling the client by name and observing a response is insufficient because patients may respond reflexively even if misidentified. This method does not fulfill the requirement of using two independent identifiers to ensure safe medication administration.

Complete proper documentation of the medication error in the patient's chart: Documentation is an essential component of error management, but it is not the immediate priority. Patient safety takes precedence, and assessment must occur first to determine whether harm has occurred before completing documentation or incident reporting.

Alert the charge nurse that a medication error has occurred: Notifying the charge nurse is appropriate after assessing the patient. However, reporting without first determining the patient’s current condition delays critical clinical evaluation and potential interventions.

Administer the antidote to the patient immediately: Administering an antidote without confirming that the wrong medication was given or that adverse effects are present could cause unnecessary harm. Treatment decisions must be based on assessment findings and verification of the specific error involved.

The Doctor's order: The original provider order initiates the medication therapy, but once transcribed and verified, medication administration should be performed using the Medication Administration Record (MAR). The MAR consolidates active, verified orders and is designed specifically for safe administration checks.

Angle of insertion is always 45 degrees: The angle of insulin injection depends on needle length and the patient’s body habitus. Many insulin syringes today use short needles that are administered at a 90-degree angle. A 45-degree angle may be used in very thin individuals but is not universally required.

Site is massaged after administration: Massaging the injection site is contraindicated because it can accelerate insulin absorption unpredictably, potentially leading to hypoglycemia. Gentle pressure may be applied if needed, but rubbing the area should be avoided.

Injection must be given close to the umbilicus: Insulin should not be injected within approximately 2 inches (5 cm) of the umbilicus because absorption may be inconsistent. The abdomen is preferred for consistent absorption, but injections should be spaced appropriately away from the navel.
