Ischemic thrombosis occurs when a blood clot forms within a diseased or damaged cerebral artery, often due to atherosclerosis. This patient’s primary risk factor is atrial fibrillation rather than localized arterial plaque rupture, making a thrombotic stroke less likely than one originating from a cardiac source or distant vessel.

Hemorrhagic stroke results from the rupture of a blood vessel, leading to intracranial bleeding. While anticoagulation therapy for atrial fibrillation increases hemorrhage risk, the arrhythmia itself specifically predisposes the patient to obstructive embolic events. There is no evidence of trauma, aneurysm, or hypertensive crisis provided in the stem.

Ischemic stenosis refers to the narrowing of a blood vessel, usually over a long period due to chronic inflammation and lipid deposition. While stenosis can reduce cerebral blood flow, it is a structural vessel change rather than an acute event triggered by the stasis of blood in the heart.

Sinus bradycardia involves a heart rate of < 60 beats per minute originating from the sinoatrial node. Lidocaine would be contraindicated or ineffective here, as it can further suppress cardiac conduction. The appropriate pharmacological intervention for symptomatic bradycardia is typically atropine, which blocks vagal stimulation to increase heart rate.

Atrial fibrillation is a supraventricular tachycardia characterized by rapid, irregular atrial electrical activity. Lidocaine has little to no effect on atrial tissue or the conduction through the atrioventricular node. Treatment for this condition usually involves beta-blockers, calcium channel blockers, or Class III antiarrhythmics like amiodarone.

First-degree heart block is a delay in conduction through the atrioventricular node, resulting in a prolonged PR interval. Administering lidocaine, which slows conduction in the ventricular conduction system, could potentially worsen the block or lead to more advanced degrees of heart block, compromising the patient’s cardiac output.

Hematocrit measures the volume percentage of red blood cells in the blood. While a value of 43% is within the normal range, it is primarily an indicator of hydration status and oxygen-carrying capacity rather than a direct measurement of the nutritional efficacy or protein synthesis provided by TPN.

A white blood cell count of 8,000/uL is within the normal reference range, indicating the absence of active infection or an appropriate inflammatory response. While important for a burn patient’s recovery, it does not evaluate the metabolic or nutritional success of the total parenteral nutrition administration.

A calcium level of 9.4 mg/dL is a normal finding but is not the primary indicator used to assess the effectiveness of TPN. Calcium levels are influenced by parathyroid function, bone metabolism, and vitamin D levels, rather than the overall caloric and protein intake provided by the nutrition.

Reassessing the blood pressure would delay life-saving intervention. The clinical presentation of hypertension, bradycardia, and flushing above the level of injury strongly indicates autonomic dysreflexia. The nurse must act immediately to reduce intracranial pressure and lower the systemic blood pressure rather than repeating a confirmed measurement.

While restlessness and sweating can occur in hypoglycemia, the patient’s significant hypertension and bradycardia are classic signs of a sympathetic surge below the level of injury. Checking blood glucose is not the priority when the patient is at immediate risk for a hemorrhagic stroke or seizure.

Providing cooling blankets addresses the symptom of sweating but does not address the underlying life-threatening hypertension. Furthermore, cold stimuli can actually act as a trigger for further sympathetic nervous system activation, potentially worsening the autonomic dysreflexia instead of resolving the noxious stimulus causing the episode.

While a patient may be slow to respond or physically exhausted during the postictal phase, a true inability to move (paralysis) is not a standard expectation of a generalized tonic-clonic seizure. Persistent focal weakness after a seizure, known as Todd’s paralysis, is a specific neurological phenomenon rather than a generalized finding.

Paresthesia, or abnormal sensations like tingling or numbness, is more commonly associated with the aura or sensory phase of a focal seizure rather than the postictal recovery period. The postictal state is characterized more by depressed consciousness and generalized muscle soreness than by localized sensory disturbances or peripheral nerve irritations.

Vital signs during the postictal phase typically remain stable or may show transient tachycardia and hypertension due to the preceding sympathetic surge. Hypotension is not an expected finding; if present, it may indicate a different underlying etiology, such as cardiogenic shock or severe dehydration, rather than the seizure recovery itself.

While monitoring vital signs is important in the postictal phase, it is practically impossible and unsafe to obtain an accurate blood pressure or pulse oximetry reading during the active, convulsive movements of a tonic-clonic seizure. The primary focus during the ictus is safety and clinical observation of the seizure activity.

Investigating medication adherence and the circumstances surrounding the event is a critical part of the history-taking process, but this is performed after the seizure has resolved. During the active phase, the nurse must prioritize immediate clinical assessment and protection of the patient from environmental injury.

An aura is a subjective sensory experience that occurs before the loss of consciousness. Because the patient is currently in the active phase of a generalized seizure, they cannot communicate these experiences. This information is gathered retrospectively once the patient has regained full consciousness and orientation.

While pre-morbid status is a factor in recovery, it is not used solely as a predictive tool for neurological outcomes. Many variables, including the size and location of the infarct, influence recovery. The primary clinical utility of the history is to provide a benchmark for current therapy.

The purpose of obtaining a functional history is for clinical planning, not to force the patient to acknowledge limitations. In fact, many stroke patients suffer from anosognosia (lack of insight into deficits), and rehabilitation focuses on utilizing remaining strengths rather than simply highlighting what has been lost physically.

It is an overgeneralization to state that all clients can regain "much" of their functioning. Every cerebrovascular accident is unique, and the degree of neuroplasticity varies. The functional history is a tool for assessment and planning rather than a guarantee of a specific level of physical recovery.

Monitoring cholesterol levels is an important part of secondary stroke prevention and long-term risk factor management. However, in the acute 24-hour window following a stroke, metabolic labs are secondary to assessments that monitor for immediate life-threatening complications like cerebral edema or hemorrhagic transformation of the infarct.

Assessing bowel sounds is part of a standard head-to-toe assessment to monitor for paralytic ileus or constipation, especially if the patient is immobile. While necessary for general care, it does not take priority over the neurological monitoring required to detect acute changes in the patient's primary brain injury.

An echocardiogram is often performed to identify a cardioembolic source of the stroke, such as a patent foramen ovale or valvular vegetation. While diagnostic, this procedure is not a continuous nursing assessment and does not provide the real-time data needed to monitor the patient's immediate physiological stability.

Burn patients are more likely to develop metabolic acidosis rather than alkalosis. Acidosis occurs due to impaired tissue perfusion, lactic acid accumulation from anaerobic metabolism, and the loss of bicarbonate. Alkalosis would typically only occur if there was significant gastric suctioning or excessive administration of alkaline fluids.

While sodium is lost through burn exudate and shifts into the interstitial space, hyperkalemia is the more immediate and lethal electrolyte disturbance seen in the emergent phase. Sodium levels may appear low due to hemodilution during fluid resuscitation, but the priority is the massive release of intracellular ions.

Initially, a patient with extensive burns may actually show a high hematocrit (hemoconcentration) due to massive fluid shifts and plasma loss. True anemia typically develops later in the course of the injury as a result of red blood cell hemolysis and bone marrow suppression during the hypermetabolic phase.
