Continue heparin therapy and monitor platelet counts: Heparin-induced thrombocytopenia (HIT) is an immune-mediated adverse reaction in which antibodies form against the heparin–platelet factor 4 complex, leading to platelet activation and thrombosis. Continuing heparin can worsen platelet consumption and increase the risk of life-threatening thromboembolic events.

Administer platelet transfusion to manage thrombocytopenia: In HIT, thrombocytopenia results from platelet activation and consumption, not from decreased production. Transfusing platelets may exacerbate thrombosis by providing additional substrate for clot formation. Platelet transfusions are generally avoided unless there is significant active bleeding.

Administer intravenous fluids to prevent dehydration: While maintaining adequate hydration is important for overall circulatory stability, IV fluids do not address the immune-mediated platelet activation or thrombotic risk associated with HIT. The immediate threat is thrombosis, which requires stopping heparin and initiating alternative anticoagulation.

Potassium level: Serum potassium levels are important to monitor because hyperkalemia is a life-threatening complication of AKI. However, potassium can fluctuate due to medications, diet, or temporary shifts between intracellular and extracellular compartments, making it less specific for evaluating true renal recovery.

Blood urea nitrogen (BUN) level: BUN is commonly elevated in AKI due to reduced excretion of nitrogenous waste. However, BUN can also be influenced by hydration status, protein intake, and catabolic states, limiting its reliability as a sole marker of improving kidney function.

Urine volume: Increased urine output may suggest improvement, particularly during the recovery phase of AKI. However, urine volume alone does not necessarily reflect adequate filtration, as patients may produce urine without effectively clearing solutes. Filtration capacity is more accurately assessed by GFR.

Monitor daily weight: Daily weight is useful for evaluating overall fluid balance trends but is not sensitive enough for rapid assessment during the emergent phase. Fluid shifts in major burns occur quickly, requiring more immediate indicators than once-daily measurements.

Check skin turgor: Skin turgor is not a reliable indicator of fluid status in burn patients because skin integrity is compromised and edema is common. Burn injury alters the elasticity and appearance of the skin, limiting the usefulness of this assessment.

Assess mucous membranes: Mucous membrane moisture may provide general information about hydration status, but it does not accurately reflect intravascular volume in patients with significant burn injuries. Rapid capillary leak and fluid shifts require more precise monitoring parameters such as urine output.

Administer IV morphine: Restlessness and stridor in a patient with facial burns strongly suggest impending airway obstruction due to edema or inhalation injury. Administering morphine may further depress respiratory drive and mask worsening airway compromise. Pain control is important but does not address the immediate threat of airway occlusion.

Elevate the HOB: Elevating the head of the bed may help reduce facial edema and promote ventilation, but it is a supportive measure rather than a definitive intervention. It does not prevent rapid airway closure caused by burn-related swelling. Immediate airway management takes precedence.

Obtain ABG: Arterial blood gas analysis provides information about oxygenation and ventilation but does not treat airway compromise. Waiting for ABG results could delay lifesaving airway intervention. Clinical signs such as stridor and restlessness warrant immediate action to secure the airway.

Hypertension and narrowing pulse pressure: Increased intracranial pressure leads to a widening pulse pressure rather than a narrowing one. As ICP rises, systolic pressure increases while diastolic pressure may remain stable or decrease, producing a widened pulse pressure. Narrow pulse pressure is not characteristic of late ICP elevation.

Hypotension and tachycardia: Hypotension and tachycardia are more consistent with hypovolemia or shock rather than increased ICP. In elevated ICP, the body attempts to preserve cerebral perfusion by increasing systemic blood pressure, not lowering it.

Hypotension and bradycardia: This combination does not reflect the compensatory physiologic response to increased ICP. Late ICP elevation is associated with systemic hypertension, not hypotension, as the body attempts to overcome reduced cerebral perfusion pressure.

Rehabilitation needs: Rehabilitation planning depends largely on burn depth, location (such as joints or face), and functional impairment rather than solely on the percentage of total body surface area (TBSA). While TBSA influences overall severity, it does not directly determine specific long-term rehabilitation strategies.

Type of intravenous fluids required: The type of IV fluid in the acute burn phase is generally standardized, commonly isotonic crystalloids such as lactated Ringer’s solution. TBSA estimation influences the volume of fluid required rather than the specific type of fluid administered.

Respiratory needs: Respiratory management is primarily influenced by the presence of inhalation injury, facial burns, or airway involvement. The rule of nines does not directly assess airway damage but estimates external burn extent.

Prepare the patient for platelet transfusion: In immune thrombocytopenic purpura (ITP), platelet destruction is immune mediated, and transfused platelets are often rapidly destroyed unless there is severe or life-threatening bleeding. Platelet transfusion is not the first intervention for petechiae and ecchymoses without active hemorrhage. Immediate safety measures take priority.

Encourage active range-of-motion exercises: Vigorous activity may increase the risk of bruising or internal bleeding in patients with thrombocytopenia. While mobility is important, activity should be limited or modified until platelet levels are stabilized. Preventing trauma is more urgent than promoting exercise at this stage.

Administer aspirin for discomfort: Aspirin inhibits platelet aggregation by irreversibly blocking cyclooxygenase-1 (COX-1), impairing thromboxane A2 production. In a patient with already reduced platelet numbers, aspirin significantly increases bleeding risk and is contraindicated.

Apply dressings to burned areas: Wound care is important to reduce infection risk and protect damaged tissue, but in electrical burns, internal injuries may be more severe than external findings. Covering the wounds does not address immediate life-threatening complications such as cardiac dysrhythmias.

Assess for pain at contact points: Pain assessment is a routine component of burn care; however, electrical injuries may cause deep tissue damage with minimal external pain initially. Life-threatening cardiac complications take precedence over pain evaluation in the primary survey.

Start two large bore IVs: Establishing large-bore IV access is essential for fluid resuscitation and medication administration. However, dysrhythmias can occur immediately after electrical exposure, making cardiac monitoring the first priority to detect and treat potentially fatal conduction abnormalities.

Low protein, low carb: A diet low in protein and carbohydrates does not meet the increased metabolic demands of burn patients. Protein is essential for tissue repair and immune function, while carbohydrates provide energy to prevent catabolism of lean body mass.

High carb, low protein: While carbohydrates provide energy, inadequate protein intake impairs wound healing and tissue regeneration. A diet emphasizing carbohydrates without sufficient protein does not support the critical anabolic processes needed for burn recovery.

Low sodium, low protein: Restricting protein in burn patients delays wound healing and compromises immune response. Sodium restriction may be indicated in certain fluid balance scenarios but is not a priority for promoting tissue repair. Adequate protein and caloric intake remain the primary nutritional focus.
