Milk and dairy products: Milk, yogurt, and other dairy products are naturally low in purines and are not associated with an increased risk of uric acid stone formation. In fact, low-fat dairy products may have a protective effect by enhancing urinary calcium binding within the gastrointestinal tract and improving uric acid metabolism. Therefore, restricting dairy intake is unnecessary for patients with uric acid renal calculi unless another medical condition warrants dietary modification.

Spinach, chocolate, and tea: Spinach, chocolate, and tea contain significant amounts of oxalates rather than purines and are primarily associated with the development of calcium oxalate renal calculi. Excessive oxalate intake can increase urinary oxalate excretion, facilitating calcium oxalate crystal formation in susceptible individuals. These foods are not the principal dietary contributors to uric acid stone formation and therefore are not the primary foods that should be avoided.

Legumes and dried fruits: Legumes contain moderate amounts of purines, but they are generally considered healthier plant-based protein sources and have a substantially lower impact on serum uric acid levels than animal-derived purines. Dried fruits are not significant sources of purines and do not directly contribute to uric acid stone formation. Current dietary recommendations emphasize limiting high-purine animal proteins rather than routinely eliminating legumes or dried fruits from the diet.

Avoid dietary sources of calcium: Restricting dietary calcium is no longer routinely recommended for most individuals with renal calculi, particularly those with calcium oxalate stones. Adequate dietary calcium binds oxalate within the gastrointestinal tract, reducing intestinal oxalate absorption and decreasing urinary oxalate excretion. Excessive calcium restriction may paradoxically increase the risk of calcium oxalate stone formation while also contributing to reduced bone mineral density and osteoporosis.

Drink diuretic fluids such as coffee: Although caffeine-containing beverages such as coffee have mild diuretic properties, they should not be relied upon as the primary means of hydration for preventing kidney stones. Excessive caffeine intake may contribute to increased fluid losses in some individuals and may not provide consistent urinary dilution if overall water intake is inadequate. The cornerstone of stone prevention remains adequate water consumption rather than beverages with diuretic effects.

Use a filter to strain all urine: Straining urine is recommended primarily during an acute episode of nephrolithiasis to capture a passed stone for laboratory analysis and determination of its chemical composition. Identifying the stone type guides future dietary modifications and pharmacological therapy but does not itself prevent stone recurrence. Once the stone has been analyzed and the acute episode has resolved, routine long-term urine straining is unnecessary and offers no protective benefit against future renal calculus formation.

Monitor temperature every 4 hours: Monitoring the client's temperature is important because fever may indicate the development of a urinary tract infection or obstructive pyelonephritis, both of which require urgent medical intervention. However, the client is currently experiencing severe renal colic, making pain management the highest priority according to the nursing process and Maslow's hierarchy of physiological needs. Temperature monitoring does not immediately address the client's acute distress.

Encourage oral fluid intake: Adequate hydration is an important component of preventing renal stone recurrence and may facilitate stone passage once acute symptoms begin to resolve. However, encouraging excessive oral fluid intake during severe ureteral obstruction can increase urinary pressure proximal to the obstruction, potentially worsening ureteral distention and intensifying the client's pain. Fluid intake should be individualized based on the client's clinical status and is not the immediate priority during severe renal colic.

Give antiemetics as needed for nausea: Nausea and vomiting commonly accompany renal colic because intense visceral pain stimulates autonomic nervous system responses and the vomiting center. Administering antiemetics helps reduce gastrointestinal symptoms, prevents dehydration, and improves patient comfort. However, nausea is secondary to the severe pain, and controlling the underlying pain often significantly reduces associated gastrointestinal symptoms, making analgesic administration the priority intervention.

Blood pressure (BP) is 102/50 mmHg: A blood pressure of 102/50 mmHg is slightly below normal but may be expected in the immediate postoperative period depending on the client's baseline blood pressure, anesthetic effects, analgesic administration, and overall volume status. This finding should continue to be monitored for trends and associated signs of hypovolemia such as tachycardia or altered mental status. Although important, this isolated blood pressure reading is less immediately concerning than persistent oliguria, which directly reflects renal perfusion and kidney function.

Decreased breath sounds at both lung bases: Diminished breath sounds at the lung bases are common after abdominal or flank surgery because postoperative pain limits deep inspiration, resulting in shallow breathing and bibasilar atelectasis. Nursing interventions such as incentive spirometry, coughing, deep-breathing exercises, early ambulation, and adequate pain management are appropriate to improve lung expansion. While this assessment finding requires intervention to prevent pulmonary complications, it is not as urgent as evidence of significantly decreased renal output.

Incisional pain level is 8/10 (10-point scale): Severe postoperative incisional pain is expected after nephrectomy because the procedure involves significant tissue dissection through the abdominal or flank musculature. Although pain should be treated promptly with prescribed analgesics to promote comfort, respiratory effort, and early mobilization, it is an anticipated postoperative finding. Unless accompanied by other abnormal signs such as excessive bleeding or wound complications, severe pain does not take priority over persistent oliguria.

I will need to take prophylactic antibiotics to prevent any urinary tract infections: Routine prophylactic antibiotic therapy is not recommended for clients performing intermittent self-catheterization because it promotes antimicrobial resistance and increases the likelihood of opportunistic infections without significantly reducing the incidence of catheter-associated urinary tract infections. Antibiotics should only be prescribed when there is confirmed symptomatic bacteriuria based on clinical findings and urine culture results. Preventing infection is primarily achieved through proper catheterization technique, hand hygiene, and adequate catheter maintenance rather than continuous antibiotic use.

I will use a sterile catheter and gloves for each time I self-catheterize: Sterile technique is generally reserved for acute care settings, hospitalized patients, or individuals with significant immunocompromise. In the home environment, clean intermittent catheterization is considered safe, effective, and evidence-based, requiring clean hands, a properly cleaned catheter, and clean handling techniques rather than sterile gloves and sterile equipment for every catheterization. Using sterile supplies each time would unnecessarily increase cost without providing additional clinical benefit for most clients.

I will need to buy seven new catheters weekly and use a new one daily: Most clients performing clean intermittent catheterization can safely reuse catheters after thoroughly cleaning, rinsing, drying, and storing them according to healthcare provider recommendations and manufacturer guidelines. Routine replacement of the catheter every day is unnecessary for most reusable catheter systems and would substantially increase healthcare costs. Catheters should instead be replaced when they become cracked, damaged, discolored, difficult to clean, or otherwise unsuitable for continued use.

Foul-smelling urine: Foul-smelling urine is a common manifestation of a urinary tract infection and results from bacterial metabolism and the presence of inflammatory cells within the urinary tract. Although this finding supports the diagnosis of pyelonephritis and should be documented, it does not indicate immediate clinical deterioration or life-threatening complications. The priority is identifying signs of systemic infection or impaired perfusion that require urgent intervention.

Temperature 37.8°C: A temperature of 37.8°C represents a low-grade fever and is a relatively expected finding in a client with acute pyelonephritis. Patients with upper urinary tract infections frequently develop temperatures exceeding 38°C accompanied by chills and malaise due to the systemic inflammatory response. While temperature trends should be monitored closely to evaluate treatment response, this finding alone is not as urgent as evidence of cardiovascular instability.

Complaint of flank pain: Flank pain is a hallmark clinical manifestation of acute pyelonephritis and occurs because inflammation of the renal parenchyma and renal capsule stimulates sensory nerve endings. It is commonly associated with costovertebral angle tenderness and may radiate toward the abdomen or groin. Although pain requires prompt assessment and analgesic management, it is an anticipated symptom and is less immediately concerning than signs of circulatory compromise.

Irrigating the catheter daily with sterile saline: Routine irrigation of an indwelling urinary catheter is not recommended because it disrupts the closed drainage system and increases the opportunity for microorganisms to enter the urinary tract. Catheter irrigation should only be performed when specifically prescribed, such as to relieve catheter obstruction caused by blood clots or debris following urological surgery. Daily irrigation does not reduce CAUTIs and may actually increase the risk of infection.

Using clean technique during insertion: Indwelling urinary catheter insertion requires strict sterile (aseptic) technique rather than clean technique to minimize the introduction of microorganisms into the normally sterile urinary tract. Using only clean technique during catheter insertion significantly increases the risk of bacterial contamination and subsequent urinary tract infection. Although clean technique is appropriate for intermittent self-catheterization in the home setting, sterile technique is the standard of care for indwelling catheter insertion.

Replacing the catheter every 7 days: Routine scheduled replacement of indwelling urinary catheters every seven days is not recommended because catheter changes themselves increase urethral trauma and create additional opportunities for bacterial introduction. Current evidence-based guidelines recommend replacing the catheter only when clinically indicated. Minimizing unnecessary catheter manipulation is a key strategy for reducing the incidence of CAUTIs.

Encourage ambulation: Ambulation may assist the passage of small renal calculi by utilizing gravity and body movement to facilitate stone migration through the urinary tract. However, clients experiencing severe renal colic are often unable to ambulate comfortably because of intense pain, restlessness, and nausea. Pain management should be established first before encouraging activity, making ambulation an appropriate intervention only after the client's condition has stabilized.

Strain all urine: Straining all urine is an important nursing intervention because it allows retrieval of the passed calculus for laboratory analysis to determine its chemical composition. Stone analysis guides long-term dietary recommendations, pharmacologic therapy, and recurrence prevention strategies. However, urine straining does not relieve the client's immediate symptoms or address the severe pain associated with ureteral obstruction, making it a secondary priority.

Increase fluid intake to 3 L/day: Adequate hydration is an essential component of preventing recurrent nephrolithiasis and may facilitate stone passage after acute symptoms begin to improve. During an episode of severe ureteral obstruction, however, aggressively increasing fluid intake may increase urine production proximal to the obstruction, raising intrarenal pressure and potentially worsening renal colic. Fluid administration should therefore be individualized and is not the first priority during acute severe pain.

Ineffective denial related to ineffective coping strategies: Ineffective denial occurs when a client consciously or unconsciously minimizes or rejects the reality of a health condition to the extent that it interferes with treatment or recovery. Although refusing to look at the stoma may appear to reflect denial, this behavior is more accurately explained by the client's emotional response to altered physical appearance rather than an inability to acknowledge the illness itself. There is insufficient evidence that the client is denying the existence of the surgery or refusing necessary treatment because of denial.

Anxiety related to threat to current status: Anxiety is a common emotional response following major surgery and may result from concerns about health, recovery, lifestyle changes, and uncertainty regarding the future. Clients with anxiety may appear restless, fearful, or express concerns about managing the ileal conduit independently. However, the specific behavior of avoiding the stoma and refusing participation in its care more directly reflects distress over altered physical appearance and self-concept rather than generalized anxiety.

Ineffective coping related to insufficient sense of control: Ineffective coping describes an inability to manage stressors, solve problems, or adapt appropriately to life changes and is usually evidenced by maladaptive behaviors, impaired decision-making, or failure to use available support systems. Although the client may eventually experience ineffective coping while adjusting to life with an ileal conduit, the assessment data specifically emphasize avoidance of the stoma because of its appearance.

Wipe from back to front after voiding: The correct perineal hygiene technique is to wipe from front to back, not from back to front. Wiping from back to front transfers enteric bacteria, particularly Escherichia coli, from the perianal region toward the urethral meatus, significantly increasing the risk of ascending urinary tract infection. Proper front-to-back cleansing minimizes bacterial contamination of the urethra and is a fundamental nursing teaching point for preventing recurrent UTIs.

Take antibiotics only when symptoms appear: Antibiotics should never be initiated solely on the basis of symptoms unless specifically prescribed by a healthcare provider. Self-administering antibiotics without medical evaluation may result in incomplete treatment, antimicrobial resistance, recurrent infections, and masking of more serious urinary tract pathology. Clients with recurrent UTIs should seek medical assessment for appropriate diagnosis, urine culture when indicated, and evidence-based antimicrobial therapy.

Avoid drinking more than 1 liter of fluid per day: Limiting fluid intake increases urine concentration, promotes urinary stasis, and reduces the mechanical flushing of microorganisms from the urinary tract, thereby increasing the risk of recurrent infections. Patients are generally encouraged to consume approximately 2–3 liters of fluid daily, unless contraindicated by conditions such as heart failure or chronic kidney disease. Adequate hydration promotes frequent voiding, dilutes bacterial concentration, and decreases bacterial adherence to the urothelium.
