Palpable central groove: The central groove is normally felt when palpating a healthy prostate gland. Its presence indicates a typical anatomical landmark and is not considered abnormal.

Being heart shape: The normal prostate gland is heart-shaped with two lateral lobes separated by a central groove. This shape is expected and does not indicate pathology.

Elastic and rubbery consistency: A healthy prostate typically feels smooth, elastic, and rubbery. This consistency is a normal characteristic and only becomes abnormal if the gland feels hard, nodular, or irregular.

large, firm, and fixed to the tissue: Large, fixed, and firm lymph nodes are concerning for malignancy or chronic disease. These characteristics are abnormal and require further investigation, not a normal finding in a healthy adult.

rubbery, discrete, and mobile: Rubbery, mobile lymph nodes can be seen in conditions such as lymphoma or after infections. While mobility is less concerning, rubbery enlargement is not considered a normal feature in a healthy individual.

shotty: Shotty nodes feel like small beads under the skin and are often associated with past or resolved infections. While sometimes benign, they are not the expected finding in a healthy adult without underlying pathology.

Even distribution of hair: Hair that is evenly distributed across the scalp is considered a normal finding. It does not suggest any underlying pathology and therefore is not a cause for concern during head assessment.

Symmetrical features at rest: Symmetry of facial features while the patient is at rest is a normal and expected finding. It demonstrates intact cranial nerve function and normal muscle tone.

Symmetrical features with movement: Symmetry during facial movements, such as smiling or raising eyebrows, is also considered normal. This finding suggests intact cranial nerve VII function.

Bell's palsy: Bell’s palsy involves paralysis of the facial nerve (cranial nerve VII), leading to weakness or drooping on one side of the face. It does not typically impair sensory function such as distinguishing between sharp and dull sensations.

Scleroderma with a pronounced proliferation of connective tissue in the face and cheeks: Scleroderma causes skin thickening and tightening but does not directly impair nerve sensation. While facial appearance and mobility may change, sensory function remains intact.

Frostbite with resultant paresthesia to the cheeks: Frostbite can cause temporary numbness, tingling, or pain, but it usually affects localized skin tissue and is not typically symmetrical across the entire face.

Vagus nerve: The vagus nerve runs through the neck and thorax, not near the TMJ. It is not a landmark used when assessing the joint.

Hyoid bone: The hyoid bone is situated in the anterior neck above the larynx and is unrelated to the location of the TMJ. It does not serve as a reference point in this assessment.

Mandible: The mandible forms part of the joint itself, but the landmark for palpation is anterior to the tragus, not simply the mandible. The tragus provides the specific external reference point used by clinicians.

Oval-shaped ovaries: The ovaries are not visualized during a speculum inspection of the vaginal canal. They are located deep within the pelvis and can only be assessed indirectly through bimanual examination or imaging studies.

Cervix with an irregular, enlarged os: An irregular or enlarged cervical os is more commonly observed in women who have given birth vaginally. In a nulliparous woman, the cervical os usually appears small and round rather than irregular.

Pear-shaped uterus: The uterus has a pear shape anatomically, but it is not visualized during a speculum inspection. Only the cervix at the end of the vaginal canal is directly seen with this procedure.

Fallopian tubes: The fallopian tubes extend laterally from the uterus into the pelvic cavity and are not visible during a vaginal inspection. They are located internally and cannot be seen without surgical procedures or imaging.

Ovaries: The ovaries are situated on either side of the uterus and lie deep within the pelvis. They are not visible during a vaginal exam, as they are not part of the structures that open directly into the vaginal canal.

Uterus: The uterus is a muscular organ that sits above the vagina, but only its lower part, the cervix, is visible on inspection. The body of the uterus is positioned higher up and cannot be seen during a routine vaginal exam.

No sensation: Complete absence of sensation would not be expected during sinus palpation, as light pressure typically produces some awareness of touch. A normal response is mild firmness rather than no feeling at all.

Pain sensation behind eyes: Pain behind the eyes may indicate sinus inflammation or infection, particularly of the ethmoid sinuses. This is not a normal finding during sinus palpation.

Pain during palpation: Pain elicited during palpation suggests sinus tenderness, which is usually associated with sinusitis or other pathology. This finding would not be considered normal.

Gently displace the nose to the side that is being examined: The nose is usually tilted slightly backward for examination rather than being displaced to the side. Lateral displacement is not required for proper visualization and could cause unnecessary discomfort.

Insert the speculum at least 3 cm into the vestibule: Inserting the speculum this far can cause pain and potential injury to the delicate nasal mucosa. The speculum should only be inserted slightly, just enough to visualize the structures of the nasal cavity.

Keep the speculum tip medial to avoid touching the floor of the nares: The speculum is directed slightly upward and not specifically kept medial. The goal is to avoid trauma to sensitive structures, with primary emphasis on avoiding the septum during insertion.

12: A Glasgow Coma Scale score of 12 indicates a moderate brain injury. The patient is impaired but not in a coma, as coma is defined by significantly lower scores.

24: The Glasgow Coma Scale ranges only from 3 to 15, so a score of 24 is not possible. The maximum score of 15 reflects a fully alert and oriented patient.

15: A score of 15 indicates a normal level of consciousness with full eye, motor, and verbal responses. This represents no impairment and does not meet criteria for coma.
