Ensure sterilization of nondisposable items with ethylene oxide: Ethylene oxide is a sterilizing agent used for heat-sensitive equipment, but it does not remove latex proteins from items that contain latex. If equipment contains latex, sterilization alone will not eliminate the allergenic proteins capable of triggering a reaction.

Cleanse latex ports on IV tubing with chlorhexidine before injecting medication: Disinfecting an injection port with chlorhexidine removes microorganisms but does not neutralize latex proteins. For clients with latex allergy, latex-free IV equipment should be used rather than attempting to disinfect latex components.

Wear hypoallergenic latex gloves that contain powder: Hypoallergenic latex gloves still contain latex proteins that can trigger allergic reactions. Powdered gloves further increase the risk because the powder can carry airborne latex particles that are easily inhaled or deposited on surfaces. In a latex-allergic environment, non-latex gloves such as nitrile or vinyl are required.

Ask the client to consider a direct donation: Direct donation involves receiving blood donated by a specific individual, often a relative or friend. Although this option may sometimes be offered for transfusion preferences, it does not address the client’s refusal based on religious beliefs. Suggesting this does not respect the client’s expressed decision.

Request a consultation with the ethics committee: Ethics consultations are helpful when there is uncertainty about the ethical course of action or conflict regarding decision-making capacity. In this situation, the client has clearly refused treatment for religious reasons, which is a legally protected right. Patient autonomy is already clear, so consultation is not the immediate priority.

Ask the client's family to intervene: Family members may express strong opinions about treatment decisions, but they do not have authority to override the decision of a competent adult client. Encouraging the family to intervene could place pressure on the client and undermine their autonomy. The nurse’s role is to support the client’s informed choice.

Alginate: Alginate dressings are highly absorbent dressings derived from seaweed and are typically used for wounds with moderate to heavy exudate. They are most appropriate for deeper wounds such as stage 3 or stage 4 pressure injuries, tunneling wounds, or bleeding wounds.

Gauze: Gauze dressings are considered a basic wound covering and are often used for packing deeper wounds or covering secondary dressings. They do not maintain a moist wound environment effectively and can adhere to the wound bed, potentially causing trauma to newly forming tissue during removal.

Transparent: Transparent film dressings are thin, adhesive coverings that allow visualization of the wound and are primarily used for superficial wounds with minimal drainage, such as stage 1 pressure injuries or areas at risk for breakdown. Stage 2 pressure injuries produce exudate, transparent films may not provide sufficient absorption or protection.

Document the provider's statement in the medical record: Accurate documentation of the provider’s instructions and the client’s condition is important for legal and clinical communication purposes. Documentation alone does not address the immediate risk to the client. When a client is showing signs of hemorrhagic shock, prompt escalation is necessary.

Complete an incident report: Incident reports are used for internal quality improvement and risk management after an event has occurred or when a significant safety issue arises. Completing an incident report does not provide an immediate solution to the client’s unstable condition. The priority in this situation is advocating for the client and escalating concerns.

Consult the facility's risk manager: Risk managers are typically involved in analyzing adverse events, legal issues, or systemic safety concerns after the situation has been stabilized. Contacting the risk manager does not provide timely clinical intervention for a patient who may be actively deteriorating from hemorrhagic shock.

Suction the client's airway: Airway suctioning is indicated when there are signs of airway obstruction such as gurgling sounds, visible secretions, ineffective coughing, or decreased oxygen saturation due to mucus accumulation. Without evidence of secretion retention, suctioning is invasive and not the most immediate or least restrictive intervention.

Instruct the client to perform incentive spirometry every hour: Incentive spirometry promotes alveolar expansion, prevents atelectasis, and improves lung ventilation in postoperative or immobilized clients. However, it is primarily a preventive and long-term respiratory intervention rather than an immediate measure for acute dyspnea.

Humidify the client's supplemental oxygen: Humidification is helpful for clients receiving prolonged oxygen therapy to prevent mucosal dryness and irritation of the respiratory tract. While it improves comfort and secretion management over time, it does not provide immediate relief for acute breathing difficulty. Therefore, it is not the priority action.

The client uses a wool blanket on their bed: Wool blankets are flammable and can easily ignite in the presence of oxygen, increasing the risk of fire. Proper oxygen safety protocol requires the client to use nonflammable materials such as cotton or synthetic flame-resistant bedding to minimize ignition hazards.

The client stores an extra oxygen tank on its side under their bed: Oxygen tanks must always be stored upright and secured to prevent tipping, rolling, or damage to the cylinder. Storing a tank on its side under a bed is unsafe because it increases the risk of leaks, accidental discharge, or physical injury if the tank falls. Proper storage protocol is essential to maintain safety.

The client has a weekly inspection checklist for oxygen equipment: Regular inspection of oxygen equipment is important for safety and functionality; however, a weekly inspection may not be sufficient depending on the client’s usage and manufacturer recommendations. Daily visual checks for leaks, tubing kinks, and tank pressure are considered best practice.

Place the client in high-Fowler's position: High-Fowler’s position (sitting at 60–90 degrees) can increase pressure on the sacrum and coccyx, which are common sites for pressure ulcers. While upright positioning may help with respiratory function, it does not prevent skin breakdown and may actually contribute to pressure-related injury if repositioning is not frequent.

Massage areas of skin that are darker than the surrounding skin tissue with unscented lotion: Massaging areas of darker or reddened skin can cause further tissue damage by disrupting already compromised capillaries. The recommended approach is to relieve pressure, maintain skin hygiene and use protective padding, rather than massaging areas at risk for breakdown.

Increase the client's intake of carbohydrates: Adequate nutrition supports overall healing, but carbohydrates alone do not directly prevent skin breakdown. Protein and vitamins (such as vitamin C and zinc) are more critical for tissue repair and maintaining skin integrity. Nutrition is a supportive measure rather than a direct preventive intervention for pressure ulcers.

"It is difficult to read the instructions because my glasses are at home.": This statement indicates a barrier to learning rather than readiness. The client may be motivated, but the lack of visual aids or necessary tools can impede effective instruction until the barrier is addressed, so teaching should be postponed or adapted accordingly.

"I'm wondering why I need to learn this.": Expressing uncertainty or lack of understanding reflects ambivalence and a knowledge gap. Before education can be effective, the nurse needs to address the client’s questions and clarify the purpose and benefits of insulin self-administration to build motivation and readiness.

"You will have to talk to my partner about this.": Deferring learning responsibility to another person suggests that the client is not ready to participate actively in their own education. Effective self-management requires direct engagement, so readiness is indicated when the client expresses willingness to learn and participate themselves.

Assign the client to a room with a negative airflow system: Negative-pressure rooms are used for airborne infections, such as tuberculosis, to prevent the spread of droplet nuclei. Clostridium difficile is transmitted via the fecal-oral route, primarily through contact with contaminated surfaces or hands, so a negative airflow system is not required. Contact precautions are sufficient

Use alcohol-based hand sanitizer when leaving the client's room: Alcohol-based hand sanitizers are ineffective against Clostridium difficile spores. Proper hand hygiene requires washing hands thoroughly with soap and water to physically remove spores, especially after contact with the client or contaminated surfaces.

Clean contaminated surfaces in the client's room with a phenol solution: Clostridium difficile spores are resistant to many common disinfectants, including phenol-based solutions. The recommended disinfectant is a bleach-based solution or EPA-approved sporicidal agent to effectively eradicate spores from surfaces and reduce infection risk.
