Administer naloxone if the client's respiratory rate is greater than 24/min: Naloxone is an opioid antagonist indicated for opioid-induced respiratory depression.. A respiratory rate greater than 24/min suggests tachypnea, which may be related to pain or anxiety. Administering naloxone in this situation could abruptly reverse analgesia and precipitate acute pain.

Inform the client to monitor for loose stools while taking opioid analgesia: Opioids decrease gastrointestinal motility by binding to mu receptors in the enteric nervous system, leading to delayed peristalsis and increased fluid absorption. The expected adverse effect is constipation rather than diarrhea.

Withhold analgesic medication unless the client reports pain: Postoperative pain management is most effective when provided on a scheduled basis or prior to activities that increase discomfort. Waiting until pain becomes severe can lead to heightened sympathetic activation, delayed mobilization, and impaired respiratory effort.

Generalized abdominal pain: Abdominal pain can occur with peritonitis due to inflammation of the peritoneal membrane and irritation of visceral and parietal structures. However, pain may develop after inflammatory processes are already established. It is not typically the earliest observable sign.

Increased heart rate: Tachycardia may occur as part of a systemic inflammatory response once infection progresses. It reflects sympathetic activation due to fever, pain, or developing sepsis. This manifestation generally appears after local signs within the peritoneal cavity are already present.

Fever: Fever develops as pyrogenic cytokines such as interleukin-1 and tumor necrosis factor act on the hypothalamus to elevate the body’s temperature set point. Although fever is a common sign of infection, it often occurs after localized peritoneal inflammation has begun. In peritoneal dialysis–associated peritonitis, changes in effluent appearance are detected before fever.

"Secure the cap tightly over the needle before you discard it.": Recapping used needles increases the risk of accidental needlestick injuries due to hand-to-needle contact. Occupational safety guidelines recommend avoiding recapping unless absolutely necessary and using a one-handed scoop technique if performed. Routine recapping prior to disposal is not advised.

"Remove the needle from the syringe before you place it in the trash.": Separating the needle from the syringe increases handling of contaminated sharps and raises the risk of accidental injury. Used needles should not be placed directly into household trash, even if detached. Safe practice requires disposing of intact sharps immediately into a puncture-resistant container.

"Place your storage container in a recycle bin when it is full.": Sharps containers should never be placed in recycling bins because this exposes sanitation workers to potential injury and biohazard contamination. Disposal must follow community guidelines for medical sharps, which often involve designated drop-off sites or approved waste programs.

Ensure suction equipment is available: Having suction equipment readily accessible is an important safety measure for clients recovering from moderate sedation because protective airway reflexes may be diminished. However, simply ensuring availability does not immediately reduce the client’s risk of aspiration. Preparation alone does not address nausea.

Auscultate bowel sounds: Assessing bowel sounds evaluates gastrointestinal motility but does not address the acute risk associated with nausea following sedation. Moderate sedation can impair gag and cough reflexes, increasing aspiration risk if vomiting occurs. Bowel assessment does not provide immediate protection against airway compromise.

Administer ondansetron: Ondansetron is an antiemetic that blocks serotonin receptors in the chemoreceptor trigger zone and can reduce nausea and vomiting. Although appropriate, medication administration takes time to prepare and act, and it does not immediately protect the airway. In a sedated client, airway protection takes precedence over symptom treatment.

Friction rub: A pleural friction rub is a grating sound heard on auscultation when inflamed pleural surfaces rub together during respiration. It is commonly associated with pleuritis or pulmonary inflammation rather than impaired gas exchange. While it indicates a pulmonary condition, it does not specifically signify respiratory failure or inadequate oxygenation.

Xerostomia: Xerostomia refers to dry mouth and is often related to dehydration, medication side effects, or reduced salivary secretion. It does not reflect impaired ventilation or oxygenation status. This finding has no direct correlation with arterial blood gas abnormalities or respiratory insufficiency.

Decreased end-tidal CO2: End-tidal carbon dioxide (ETCO2) reflects the amount of carbon dioxide exhaled at the end of expiration and helps assess ventilation. Respiratory failure due to hypoventilation results in elevated, not decreased, CO2 levels. A decreased ETCO2 is more commonly seen with hyperventilation, poor perfusion, or equipment-related issues.

Decrease traction on the catheter: Traction is commonly applied after a transurethral resection of the prostate to provide pressure on the prostatic fossa and minimize bleeding. While excessive traction can contribute to discomfort, bladder spasms accompanied by reduced drainage more strongly suggest obstruction from blood clots rather than traction-related irritation.

Remove the indwelling urinary catheter: The three-way indwelling catheter is essential for maintaining continuous bladder irrigation and preventing clot retention after prostate surgery. Removing the catheter would interrupt irrigation, increase the risk of urinary retention, and potentially worsen bleeding. Catheter removal is contraindicated unless prescribed by provider.

Administer ibuprofen 400 mg for pain relief: Although bladder spasms cause discomfort, nonsteroidal anti-inflammatory drugs do not address the underlying cause of decreased urinary output in this context. Treating pain alone without correcting possible obstruction could lead to bladder distention and further complications.

Store oxygen tanks on their side when not in use: Oxygen cylinders should be stored upright in a secured holder or stand to prevent tipping and potential damage to the valve system. If a cylinder falls and the valve breaks, it can become a high-pressure projectile due to rapid gas release. Storing tanks on their side increases the risk of rolling, impact, and mechanical damage.

Increase the oxygen flow rate by 1 L if the client experiences dyspnea: Oxygen flow rates are prescribed based on the client’s clinical condition and arterial oxygenation status. Altering the flow without a provider’s order may lead to complications such as carbon dioxide retention in clients with chronic hypercapnia.

Replace cotton blankets with wool and synthetic blankets: Wool and many synthetic fabrics can generate static electricity, which poses an ignition risk in oxygen-enriched environments. Cotton materials are preferred because they produce less static and reduce fire hazard. Maintaining low-static materials in the home supports safer oxygen use.

"Place a firm pillow under your head.": Placing a firm pillow under the head may increase cervical flexion, which can contribute to neck contractures in clients with upper body burns. Neutral positioning without excessive flexion or extension is preferred to maintain joint mobility and prevent deformity.

"Remain in a side-lying position.": Side-lying may help prevent pressure injuries, but it does not specifically address prevention of upper extremity or shoulder contractures. Positioning should focus on maintaining joints in functional alignment and preventing flexion contractures in the affected areas.

"Keep your elbow in a flexed position.": Flexing the elbow increases the risk of contracture formation and limits future extension. The nurse should instruct the client to maintain the elbow in an extended or slightly flexed functional position, depending on the burn location, to maintain mobility and prevent deformity.

Request a prescription for a nightly sedative: Routine use of sedatives in clients with dementia is not recommended due to the increased risk of falls, confusion, and respiratory depression. Nonpharmacological interventions and environmental modifications are preferred strategies for promoting safety and sleep without compromising cognitive function.

Turn off all lights in the client's room at night: Complete darkness may increase confusion or agitation in clients with dementia, especially those prone to sundowning. Providing low-level night lighting helps the client orient themselves, reduces the risk of falls, and supports safer nighttime mobility.

Assist the client with toileting at least once every 4 hr: Regular toileting schedules are important, but assistance every 4 hours may not meet the client’s individual needs. Dementia care typically involves prompting or assisting the client to use the toilet based on observed cues, urgency, or a more frequent schedule, rather than a rigid 4-hour interval.

"I'll make sure I take these tablets on an empty stomach.": Montelukast can be taken with or without food. Taking it on an empty stomach is not necessary and does not improve absorption or efficacy. Patient education should focus on daily adherence rather than timing with meals.

"Once I take this for a while, I won't need my inhaler anymore.": Montelukast is not a rescue medication and does not provide immediate relief during acute asthma attacks. Clients may still require short-acting bronchodilators, such as albuterol inhalers, for sudden symptoms.

"I'll take this medication to stop wheezing when I have an attack.": Montelukast does not act quickly enough to relieve acute bronchospasm. Using it as a rescue therapy is ineffective and potentially dangerous if it delays appropriate treatment with a rapid-acting bronchodilator.
