Methotrexate for rheumatoid arthritis is administered once weekly, not daily. Taking it daily can lead to severe toxicity, including bone marrow suppression, increased risk of infection, hepatotoxicity, and gastrointestinal ulceration. This makes daily dosing a dangerous and common medication error.

Avoiding foods high in vitamin K is relevant for clients taking anticoagulants such as Warfarin, not methotrexate. Methotrexate does not interact with vitamin K in this way. Instead, clients are often prescribed folic acid supplements to reduce adverse effects like stomatitis and bone marrow suppression.

Methotrexate can cause stomatitis and oral mucositis due to its effect on rapidly dividing cells. Alcohol-based mouthwashes can irritate and dry the oral mucosa, worsening discomfort and increasing the risk of ulceration. Clients should use non-alcohol-based rinses such as saline or baking soda solutions instead.

Although Diphenhydramine may be administered to treat allergic symptoms, it is not the priority initial action. The medication must first be stopped and the client assessed to prevent worsening of the reaction.

Notifying the healthcare provider is appropriate, but it is not the first action. Immediate nursing intervention to stop the drug and assess the client takes priority.

Continuing the medication despite signs of an allergic reaction is unsafe and could lead to severe anaphylaxis. The medication must be discontinued immediately.

Mild nausea is a relatively common gastrointestinal side effect of NSAIDs, including ketorolac. It occurs due to irritation of the gastric mucosa and prostaglandin inhibition. While it should be monitored and managed (e.g., administering with food if appropriate or using antiemetics), it does not require discontinuation unless it progresses to severe symptoms such as persistent vomiting or signs of gastrointestinal bleeding.

Mild lower-extremity edema can occur with NSAIDs due to fluid retention and altered renal prostaglandin activity. Although it should be assessed and monitored, especially in clients with cardiac or renal conditions, mild edema alone is not an immediate reason to discontinue the medication unless it worsens or is accompanied by signs of renal dysfunction or heart failure.

A decrease in pain level from 8/10 to 4/10 demonstrates that the medication is providing effective analgesia. This is an expected therapeutic outcome and does not indicate a need to stop the medication.

Ignoring side effects is unsafe. Aspirin toxicity can escalate rapidly, and early recognition and intervention are critical. Side effects such as gastrointestinal bleeding, tinnitus, or altered mental status should never be ignored.

Aspirin toxicity usually causes hyperventilation (increased respiratory rate) rather than slowed breathing, as the body attempts to compensate for metabolic acidosis caused by salicylate accumulation.

Constipation is not a common manifestation of aspirin toxicity. Gastrointestinal effects from aspirin typically include dyspepsia, nausea, vomiting, and gastrointestinal bleeding, not constipation. Increasing fiber intake would not address salicylate toxicity symptoms.

Mild edema may occur with NSAID use due to fluid retention but is not an immediate reason to discontinue ketorolac unless it worsens or is associated with other complications such as heart failure or kidney dysfunction.

A decrease in pain level indicates that the medication is effective, which supports its therapeutic use rather than questioning it.
