Educating the client about postoperative expectations is a necessary nursing intervention to reduce anxiety and improve recovery outcomes. However, this is not the priority action in the preoperative area when compared to patient safety protocols. Education should occur only after the nurse has ensured that the correct patient is prepared for the correct procedure.

Introducing the client to surgical team members helps build rapport and facilitates communication within the perioperative environment. While this supports a positive patient experience, it does not address the immediate safety requirements of the preoperative phase. Safety verification must always precede social introductions or non-emergent team coordination to ensure the clinical pathway is accurate for the individual.

Obtaining a full set of vital signs is a vital component of the preoperative assessment to establish a baseline for intraoperative monitoring. Although clinically significant, it is secondary to the primary safety goal of verifying the patient and site. If the nurse collects data on the wrong patient, the subsequent surgical process remains inherently unsafe despite having accurate physiological measurements.

The Recommendation section of SBAR is reserved for suggesting specific actions, immediate needs, or the desired plan of care for the patient. It focuses on what should happen next, such as the frequency of vital signs or specific medication orders. Comorbidities like diabetes and hypertension are historical facts rather than future-oriented suggestions for the incoming nurse's immediate action.

The Situation section is a concise statement that identifies the nurse, the patient, and the most immediate reason for the communication. In a PACU hand-off, this would typically involve stating the patient's name and the specific procedure they just completed. It does not include long-term medical history or chronic comorbidities, as those details would dilute the urgency of the immediate situation statement.

The Assessment section involves communicating the nurse's professional conclusion regarding the patient's current status based on clinical findings and observations. This includes recent vital signs, pain levels, and physical exam results obtained during the previous shift or procedure. Chronic history and established comorbidities are considered pre-existing data rather than findings from the nurse's current, active assessment of the patient.

Sanguineous drainage represents active bleeding or fresh bloody discharge from an open wound or incision. While it may accompany an injury, it is not one of the five cardinal signs of the localized inflammatory response. Inflammation can occur in closed tissue injuries, such as a sprain, where no external drainage is present but internal cellular changes are occurring.

Palpable pedal pulses indicate adequate arterial perfusion to the distal extremity but are not a result of the inflammatory process itself. In cases of severe localized inflammation and edema, pressure on the vasculature can actually diminish the strength of distal pulses. While the nurse should assess for pulses, their presence is a sign of normal circulatory function rather than an inflammatory manifestation.

A full range of motion is typically lost during localized inflammation due to pain and swelling (tumor) within the joint or surrounding soft tissue. Edema increases interstitial pressure, which mechanically restricts movement, while inflammatory mediators sensitize nociceptors to produce pain during activity. Expecting a full range of motion in a newly injured, inflamed ankle is clinically inconsistent with inflammatory pathophysiology.

This statement indicates a misunderstanding, as systemic lupus erythematosus (SLE) is a multisystem autoimmune disease that can damage the kidneys, heart, lungs, and joints. The type of lupus that only affects the skin is known as discoid lupus erythematosus (DLE). Understanding that SLE has systemic implications is crucial for the patient to monitor for diverse symptoms beyond dermatological changes.

While low-grade fevers can occur during a lupus flare-up, they are not a normal daily expectation for a stable patient. A new or persistent fever may indicate an infection or an impending exacerbation of the disease, requiring immediate medical evaluation. Patients should be taught that fever is a significant clinical sign rather than a benign, everyday occurrence of the condition.

Patients with SLE are often highly photosensitive, and SPF 15 is generally considered insufficient protection against UV rays that can trigger systemic flares. Clinical guidelines typically recommend a broad-spectrum sunscreen with an SPF of at least 30 or 50, even on cloudy days. The statement fails to reflect the necessary level of sun protection required to prevent immune system activation.

This breakfast choice is high in fiber and healthy fats but lacks significant sources of vitamin B12. Vitamin B12 is found almost exclusively in animal products or specially fortified foods; peanuts, whole grains, and oranges do not naturally contain the vitamin. This selection would not help a patient increase their B12 intake to correct a nutritional deficiency.

Oatmeal, almond milk, and bananas are plant-based foods that do not provide vitamin B12 unless they have been heavily fortified by the manufacturer. While healthy, this meal is largely inadequate for a patient needing to resolve a B12 deficit. Patients relying on plant-based diets must be specifically taught to look for labels indicating added cobalamin to meet their needs.

Soy yogurt and granola are typically plant-derived and are not natural sources of vitamin B12. While some soy products are fortified, they are less reliable than the animal-based proteins often recommended for correcting a clinical deficiency. This choice does not consistently guarantee the high levels of cobalamin required for a patient undergoing dietary management of B12 deficiency.

Assisting the client with a dental appointment addresses oral hygiene but fails to identify the underlying systemic cause of the bleeding. In a patient taking methotrexate, bleeding gums are often a clinical manifestation of drug-induced bone marrow suppression rather than primary dental disease. Delaying a clinical investigation to focus on dentistry could allow a life-threatening hematologic crisis to go untreated.

Disinfecting the toothbrush is an appropriate measure to prevent secondary infections in immunocompromised patients, but it does not address the physiological cause of the hemorrhage. While infection control is important, the nurse must prioritize diagnostic assessment over routine hygiene instructions when a new symptom of toxicity appears. This action provides no data regarding the patient's current hematologic stability or safety.

Educating the client on iron-rich meals might eventually help if the patient develops iron-deficiency anemia due to chronic blood loss. However, it is an inappropriate immediate response to an acute report of bleeding, as it does not address the likely mechanism of thrombocytopenia. The nurse must first determine why the patient is bleeding before implementing long-term nutritional interventions that do not provide acute stabilization.

Severe hyperglycemia, specifically associated with diabetic ketoacidosis, leads to the overproduction of ketone bodies, which are organic acids. This pathological state results in metabolic acidosis as the blood pH drops and bicarbonate is consumed to buffer the excess hydrogen ions. It is the physiological opposite of metabolic alkalosis and would present with a low pH and low bicarbonate levels.

Prolonged diarrhea causes the significant loss of alkaline intestinal secretions and bicarbonate through the lower gastrointestinal tract. This depletion of base results in a relative excess of acid within the systemic circulation, leading to metabolic acidosis. Diarrhea is a classic clinical cause of a decreased serum pH and is not associated with the development of an alkalotic state.

Chronic kidney disease generally leads to metabolic acidosis because the failing kidneys are unable to excrete hydrogen ions or regenerate sufficient bicarbonate. The accumulation of metabolic waste products and phosphates further acidifies the blood as the glomerular filtration rate declines. Alkalosis is highly unusual in renal failure unless the patient is receiving excessive exogenous bicarbonate or undergoing specific therapeutic interventions.

While an increased serum sodium level, or hypernatremia, requires clinical attention, it is not a specific contraindication or a primary adverse effect associated with gentamicin therapy. Gentamicin does not directly interfere with sodium regulation or the endocrine mechanisms of water balance. The nurse should prioritize findings that suggest organ-specific toxicity related to the drug's known safety profile, such as renal dysfunction.

Decreased serum potassium can occur with various medications, but it is not a classic hallmark of aminoglycoside toxicity or a reason to withhold an initial dose of gentamicin. Hypokalemia would be more concerning if the patient were taking diuretics or digoxin. In the context of gentamicin, the most critical electrolyte disturbances are usually secondary to the renal tubular damage indicated by rising creatinine.

Increased blood glucose levels are typically associated with diabetes mellitus or corticosteroid use rather than aminoglycoside administration. Gentamicin does not affect insulin secretion, peripheral glucose uptake, or hepatic gluconeogenesis in a clinically significant manner. Therefore, while hyperglycemia is an important metabolic finding, it does not represent an immediate safety risk specifically tied to the administration of this antibiotic.

Continuous heparin infusions are typically reserved for the treatment of an existing, confirmed thrombus or for high-risk cardiac conditions rather than routine postoperative prophylaxis. This form of anticoagulation requires intensive monitoring of the activated partial thromboplastin time (aPTT) and carries a higher risk of surgical site bleeding. It is not the standard first-line choice for preventing a DVT after an orthopedic procedure.

Alteplase is a thrombolytic agent, or "clot buster," used to dissolve established, life-threatening thrombi in cases of ischemic stroke or massive pulmonary embolism. Using it prophylactically after a total knee replacement would be extremely dangerous and likely cause catastrophic postoperative hemorrhage. Thrombolytics work by activating plasminogen and are never used as a standard preventative measure for venous thromboembolism.

While aspirin has antiplatelet properties and is sometimes used in specific long-term orthopedic protocols, it is generally considered less potent than enoxaparin for preventing venous thromboembolism. Aspirin primarily targets arterial clot formation rather than the venous stasis issues seen after surgery. Most acute care protocols prioritize pharmacological anticoagulants like enoxaparin to ensure the highest level of protection against deep vein thrombosis.

An alkalotic blood pH is defined as any value greater than 7.45. The client's pH of 7.36 falls within the normal physiological range of 7.35 to 7.45, although it is on the more acidic side of the spectrum. Interpreting this result as alkalotic is a clinical error and ignores the standard reference ranges for arterial blood gas analysis.

The fact that the pH is 7.36 (within normal limits) despite a significantly elevated PaCO2 suggests that the body has successfully compensated for the imbalance. In respiratory acidosis, the kidneys retain bicarbonate to buffer the excess acid and return the pH to a normal range. If the client were unable to compensate, the pH would be significantly below 7.35, indicating a decompensated state.

In a state of respiratory acidosis with normal pH, the body would actually have an excess or normal level of bicarbonate rather than a deficiency. The kidneys increase bicarbonate reabsorption to neutralize the high levels of carbonic acid produced by the retained CO2. A deficiency of bicarbonate would cause metabolic acidosis, which would typically result in a low pH and a low PaCO2 as the lungs try to compensate.
