Hypocalcemia, defined as a serum calcium level below 9.0 mg/dL (in this case 7.6 mg/dL), typically causes a prolongation of the QT interval on an electrocardiogram, not a shortening. A prolonged QT interval occurs because the phase 2 of the cardiac action potential is extended due to the effect of low calcium on the myocardial cells. Constipation is a clinical manifestation of hypercalcemia, which slows gastrointestinal motility and smooth muscle contraction. Thus, the nurse would expect to see depleted glycogen stores rather than an increase in storage due to the insulin deficit.

Warfarin is an oral anticoagulant that inhibits vitamin K dependent clotting factors, including factors II, VII, IX, and X. While effective for long term anticoagulation, it has a slow onset of action, often taking several days to reach a therapeutic International Normalized Ratio of 2.0 to 3.0. In the immediate postoperative period, rapid acting prophylaxis is preferred. Warfarin also requires frequent blood monitoring and has many drug interactions, making it less ideal than enoxaparin for the initial phase of postoperative DVT prevention.

Aspirin is an antiplatelet agent that inhibits cyclooxygenase 1, thereby preventing the synthesis of thromboxane A2. While it reduces platelet aggregation, it is generally considered less potent than anticoagulants like enoxaparin for preventing venous clots in high risk surgical patients. Current clinical guidelines often prioritize anticoagulants over antiplatelet monotherapy for DVT prophylaxis following major orthopedic procedures like hip replacement. It may be used in specific low risk scenarios, but enoxaparin remains the standard for immediate postoperative care.

Ondansetron is a selective serotonin 5-HT3 receptor antagonist used specifically for the prevention and treatment of postoperative nausea and vomiting. It has no effect on the coagulation cascade or platelet function. Therefore, it provides no protection against the development of deep vein thrombosis or pulmonary embolism. While it is a common medication administered in the recovery room to improve patient comfort, it does not fulfill the requirement for pharmacological thromboprophylaxis after a hip arthroplasty procedure.
 

Educating the client about expectations is an important part of the perioperative process to reduce anxiety and improve postoperative outcomes. Since this value is normal, it does not represent an adverse outcome or a complication of the treatment being provided for hyperkalemia. The nurse would consider this a stable and expected laboratory finding.

A calcium level of 10.0 mg/dL is within the normal reference range of 9.0 to 10.5 mg/dL. Calcium is essential for myocardial contractility, nerve impulse transmission, and blood clotting. While intravenous calcium gluconate is often given in hyperkalemia to stabilize the cardiac membrane, the administration of insulin itself does not usually cause fluctuations in serum calcium levels. Therefore, a normal calcium result is not an adverse outcome, but rather an indication of electrolyte stability in the client.

A serum potassium of 4.8 mEq/L is within the normal range of 3.5 to 5.0 mEq/L. In a client being treated for hyperkalemia, which is defined as a potassium level greater than 5.0 mEq/L, this result indicates that the insulin therapy has been successful in lowering the potassium to a safe level. This is the desired therapeutic effect of the medication, not an adverse outcome. The nurse should continue to monitor the patient but can be reassured by this normalization.
