1230: This is earlier than the usual NPH peak for a 0700 dose; it might help some patients but is less likely to coincide with the NPH peak than 1500.

0730: Too soon after a 0700 NPH injection - NPH has a delayed onset/peak, so a snack at 0730 is unnecessary and could cause hyperglycemia.

0900: Also early (about 2 hours post-dose) - not the typical NPH peak time, so not the most appropriate time to plan the snack.

Place the client on bedrest: Myxedema (severe hypothyroidism) does not automatically require strict bedrest; activity as tolerated and monitoring are usual.

Limit high-fiber foods: Hypothyroidism commonly causes constipation, so high-fiber foods are generally encouraged to help bowel function (note: separate timing of fiber and levothyroxine administration if medication absorption is a concern).

Check the client for weight loss: Hypothyroid clients more commonly have weight gain; monitoring for weight gain (not loss) is appropriate.

Stress incontinence, vomiting, and edema: These are not the classic triad for diabetes insipidus.

Bradycardia, insomnia, and muscle cramps: Not characteristic of DI.

Dizziness, hypertension, and excitability: DI more often causes hypotension or orthostatic symptoms from volume loss, not sustained hypertension or excitability.

"I will increase the intensity of my exercise routine if my urine is positive for ketones.": Presence of urinary ketones indicates poor glucose control/possible ketosis; exercise is contraindicated because it can worsen ketoacidosis.

"I should exercise first thing in the morning before eating breakfast.": Exercising in the fasting state increases hypoglycemia risk unless insulin/food adjustments are planned.

"I will carry a complex carbohydrate snack with me when I exercise.": When hypoglycemia is a risk during exercise, patients should carry a fast-acting (simple) carbohydrate (juice, glucose tablets, candy), not a complex carbohydrate which acts more slowly.

Impaired skin integrity: Hyperparathyroidism does not primarily cause skin breakdown; skin changes are not a defining complication of elevated parathyroid hormone.

Dysphagia: Swallowing problems are not a common manifestation of hyperparathyroidism; enlargement of the parathyroid glands rarely produces compressive symptoms.

Fluid retention: Elevated parathyroid hormone and hypercalcemia more commonly lead to polyuria and dehydration rather than fluid retention.

"Test your blood glucose level every 6 hours.": During illness blood glucose monitoring is generally more frequent (for example every 3–4 hours) to detect rapid changes and guide treatment.

"Administer your usual daily dose of insulin.": Insulin should not be withheld during illness; however dosing may need adjustment based on more frequent glucose checks and oral intake - continuation of baseline insulin is emphasized but individualized.

"Limit juices, soda, and gelatin.": When at risk of hypoglycemia or unable to eat normally, easily absorbed carbohydrate-containing fluids (juice, soda) are often recommended to treat low blood glucose; blanket restriction of these during illness is not standard sick-day teaching.

Diaphoresis: Excessive sweating is more associated with an overactive thyroid state rather than hypothyroidism.

Protruding eyeballs: Exophthalmos is classically associated with Graves’ disease (hyperthyroidism), not hypothyroidism.

Palpitations: Rapid heart rate and palpitations are typical of hyperthyroidism; hypothyroidism tends to slow cardiac rate.

Moist mucous membranes: Diabetes insipidus causes large-volume dilute urine and polyuria, which typically results in dry mucous membranes from fluid loss.

Bounding peripheral pulses: Volume depletion from DI usually produces weak, thready pulses rather than bounding pulses.

Bradycardia: Fluid loss and hypovolemia more commonly cause tachycardia as a compensatory response.
