Ability to achieve independent transfer from bed to wheelchair: Independent transfers require strong upper extremity function, particularly triceps extension and wrist stability, which are typically intact at C7 or below. With a C5 injury, triceps function is absent, significantly limiting the ability to bear weight through the arms for safe independent transfers.

Independent control of bowel and bladder function: A complete spinal cord injury above the sacral segments disrupts voluntary control of bowel and bladder function. Although reflex (spastic) bladder and bowel activity may occur, conscious control is lost. Management usually involves scheduled toileting programs and catheterization rather than independent control.

Use of a wheelchair with a chin or mouth stick: Chin or mouth stick controls are typically required for clients with higher cervical injuries, such as C1–C4, where upper extremity movement is absent. At the C5 level, the client retains some shoulder and elbow function, allowing use of a powered wheelchair with hand controls or limited manual mobility.

Investigate the client's emotional concerns: Ulcerative colitis is a chronic inflammatory bowel disease that can significantly affect psychosocial well-being. Addressing emotional concerns is important. However, during an acute exacerbation, physiologic instability and complications such as dehydration or electrolyte imbalance take priority over psychosocial assessment.

Check the client's perianal skin integrity: Frequent bloody diarrhea during an exacerbation increases the risk of skin breakdown and irritation in the perianal area. Assessment of skin integrity is an important nursing intervention to prevent secondary complications. Nevertheless, this is not the immediate priority when compared to potential systemic imbalances.

Obtain a dietary history from the client: Nutritional assessment is essential in managing ulcerative colitis, particularly to identify trigger foods and prevent malnutrition. However, dietary history is not the immediate priority during an acute flare when fluid and electrolyte stability must be ensured first. Stabilization precedes detailed nutritional evaluation.

Clinically isolating syndrome: Clinically isolated syndrome refers to a first episode of neurologic symptoms lasting at least 24 hours caused by inflammation or demyelination in the central nervous system. The pattern described involves repeated periods of symptom activity followed by remission, which extends beyond a single isolated event.

Secondary progressive multiple sclerosis: Secondary progressive multiple sclerosis begins as relapsing-remitting disease but transitions into a phase of steady neurologic decline with or without superimposed relapses. The hallmark is progressive worsening rather than clearly defined symptom-free intervals.

Primary progressive multiple sclerosis: Primary progressive multiple sclerosis is characterized by gradual neurologic decline from disease onset without distinct relapses or remissions. Symptoms steadily worsen over time due to ongoing demyelination and axonal loss. The absence of symptom-free periods distinguishes it from the pattern described.

"I will reduce my fluid intake with this medication.": Adequate hydration is important while taking mesalamine because the medication is excreted renally and can, in rare cases, contribute to nephrotoxicity. Decreasing fluid intake may increase the risk of renal complications and concentrated urine. Clients are generally encouraged to maintain appropriate hydration.

"I will take this medication with ibuprofen for better pain control.": Nonsteroidal anti-inflammatory drugs such as ibuprofen can irritate the gastrointestinal mucosa and may exacerbate inflammatory bowel disease symptoms. Concurrent use may increase the risk of GI bleeding or worsening inflammation.

"I should crush this medication so I can take it easily.": Many mesalamine formulations are enteric-coated or delayed-release to ensure delivery of the drug to specific areas of the intestine. Crushing the tablet disrupts this protective coating and alters drug release, reducing therapeutic effectiveness and increasing gastric irritation.

Cleanse with antimicrobial scrub and thoroughly dry: Antimicrobial scrubs are often harsh and can strip the skin of natural protective oils, increasing irritation and breakdown. Frequent diarrhea already exposes the perianal skin to digestive enzymes and bile salts, which compromise the epidermal barrier. Vigorous cleansing and aggressive drying may further damage tissue.

Soak in a sitz bath for 20 min after each stool: While sitz baths can provide comfort and improve hygiene, prolonged and repeated soaking may lead to excessive moisture exposure and skin maceration. Continuous wetness weakens the epidermal barrier and increases susceptibility to breakdown. Routine long-duration soaking after every stool is not optimal.

Administer an enema to cleanse the colon: Enemas stimulate bowel evacuation and can increase peristalsis, potentially worsening diarrhea and further irritating the inflamed colonic mucosa. In ulcerative colitis exacerbations, minimizing additional irritation to the bowel is essential. Enema administration would not support perianal skin protection in this context.

This medication can cause significant weight loss: Prednisone is a systemic corticosteroid that commonly causes increased appetite, fluid retention, and fat redistribution rather than weight loss. Clients may experience weight gain, moon face, and truncal obesity due to its glucocorticoid effects. Significant weight loss would not be an expected teaching point.

Increase the dosage if your symptoms of Crohn's disease worsen: Prednisone dosing must be carefully prescribed and tapered under provider supervision to avoid adrenal suppression. Abrupt dosage increases without medical guidance can lead to excessive immunosuppression and adverse metabolic effects. Clients should report worsening symptoms.

Stop taking prednisone immediately if experiencing side effects: Long-term corticosteroid use suppresses the hypothalamic-pituitary-adrenal axis, reducing endogenous cortisol production. Abrupt discontinuation can precipitate acute adrenal insufficiency, characterized by hypotension, weakness, and potential circulatory collapse. Gradual tapering is required.

Low alkaline phosphatase level: ALP is associated with hepatobiliary or bone disorders and is not characteristically decreased during a Crohn’s disease exacerbation. Inflammatory bowel disease more commonly affects markers of inflammation and nutritional status. A low ALP level does not reflect the inflammatory or bleeding complications seen in active disease.

Elevated hemoglobin: During an exacerbation, chronic intestinal inflammation and mucosal ulceration can result in blood loss through the gastrointestinal tract. Ongoing bleeding and impaired nutrient absorption often lead to iron deficiency anemia rather than elevated hemoglobin.

Increased albumin level: Albumin is a negative acute-phase reactant and often decreases during systemic inflammation. In Crohn’s disease exacerbations, malabsorption and protein loss through inflamed intestinal mucosa can contribute to hypoalbuminemia. Elevated albumin levels are not typical in the presence of active inflammatory processes.

Intestinal parasites: While parasites can cause anemia in certain populations, they are not a characteristic manifestation of ulcerative colitis. Ulcerative colitis is an autoimmune inflammatory condition confined to the colon and rectum rather than a parasitic infection. Parasitic infestation would represent a separate etiology rather than a complication of colitis.

Intestinal malabsorption syndrome: Significant malabsorption is more characteristic of Crohn’s disease, particularly when the small intestine is involved. Ulcerative colitis primarily affects the colon, where nutrient absorption is limited compared to the small intestine. Anemia in ulcerative colitis is more strongly linked to blood loss than to widespread nutrient malabsorption.

Dietary iron restrictions: Clients with ulcerative colitis are not routinely placed on iron-restricted diets. In fact, iron supplementation is often required to correct anemia. Dietary restriction alone would not explain the anemia associated with inflammatory colonic ulceration and bleeding.

Strawberry yogurt: During exacerbations, clients are advised a low-residue, low-fiber diet to reduce bowel stimulation. Strawberry yogurt may contain seeds and fruit pieces that increase mechanical irritation of the intestinal mucosa. Some clients with inflammatory bowel disease experience secondary lactose intolerance, which can worsen diarrhea and cramping.

Tossed spinach salad: Raw vegetables such as spinach are high in insoluble fiber, which increases stool bulk and intestinal motility. In Crohn’s disease, especially during active inflammation, high-fiber foods can exacerbate abdominal pain, diarrhea, and cramping. Raw leafy greens may also be difficult to digest when the intestinal mucosa is inflamed.

Buttered popcorn: Popcorn is high in insoluble fiber and contains hulls that are poorly digested. These can irritate inflamed intestinal tissue and increase peristalsis, potentially worsening symptoms such as diarrhea and abdominal discomfort. High-residue foods are generally avoided during flare-ups of Crohn’s disease.

heart failure: While severe fluid loss and electrolyte imbalances can increase cardiac workload, heart failure is not a common direct complication of ulcerative colitis exacerbations. The primary concerns are related to volume depletion, electrolyte disturbances, and systemic inflammation rather than chronic cardiac dysfunction in the acute phase.

deep vein thrombosis: Inflammatory bowel disease increases the risk of hypercoagulability, which can contribute to deep vein thrombosis. However, this is less immediate than electrolyte disturbances during an acute flare. DVT assessment is important but is not the first priority when evaluating a client with ongoing diarrhea.

hypocalcemia: Although malabsorption can eventually lead to calcium deficits, acute ulcerative colitis exacerbations rarely cause immediate hypocalcemia. Calcium loss is not directly linked to short-term diarrhea, making hypocalcemia a less likely acute complication.
