Prepare for needle thoracostomy: Needle thoracostomy is typically reserved for tension pneumothorax, where the trachea is deviated and there is severe respiratory distress. In this client, the trachea is midline and oxygenation is mildly reduced, making needle thoracostomy unnecessary at this time.

Prepare oral pain medications: While pain management is important, oral medications alone will not address the underlying life-threatening issue of blood in the pleural space. Immediate intervention to evacuate the hemothorax takes priority over analgesia.

Prepare for mechanical ventilation: Mechanical ventilation may be necessary if respiratory failure develops, but the client’s oxygen saturation of 92% and stable airway indicate that ventilation support is not immediately required. The focus should first be on resolving the hemothorax.

Respiratory rate of 10 breaths per minute: A respiratory rate of 10 is below the normal range and may indicate hypoventilation or inadequate respiratory drive, suggesting the client may not yet be ready for extubation. Adequate spontaneous breathing is critical before removing the endotracheal tube.

Oxygen saturation of 95%: While an oxygen saturation of 95% indicates adequate oxygenation, it alone does not confirm that the client can maintain airway protection or effective ventilation after extubation. Other readiness criteria must also be met.

Pink nailbeds with brisk capillary refill: These findings reflect adequate peripheral perfusion but do not provide information about airway control, respiratory effort, or the ability to protect the airway, which are essential for extubation readiness.

White blood cells (WBCs) increase from 5000 to 8000/mm³ (5 to 8 x 10⁹/L): WBC count is not directly affected by hemodialysis and reflects immune status rather than dialysis effectiveness. Changes in WBCs are unrelated to solute removal or fluid balance achieved during the procedure.

Hemoglobin increases from 10 to 12 g/dL (100 to 120 mmol/L): Hemoglobin levels do not change acutely after a single dialysis session, as erythropoiesis and anemia management are long-term processes. An immediate rise in hemoglobin is not an indicator of dialysis effectiveness.

Blood urea nitrogen (BUN) decreases from 110 to 90 mg/dL (39.3 to 32.1 mmol/L): Although BUN is reduced by dialysis, a decrease from 110 to 90 mg/dL is relatively small and may indicate incomplete removal of urea. A more significant drop is usually expected to demonstrate optimal dialysis efficacy.

Increase potassium: Clients with end-stage renal failure are at high risk for hyperkalemia because the kidneys cannot adequately excrete potassium. Increasing potassium intake could lead to dangerous cardiac arrhythmias, potassium intake is usually restricted, not increased.

Decrease calcium: Calcium levels are often low in end-stage renal failure due to impaired vitamin D activation, so calcium intake is generally monitored and supplemented as needed rather than decreased. Restricting calcium without indication can worsen bone disease associated with renal failure.

Increase phosphate: Clients with renal failure typically develop hyperphosphatemia because phosphate excretion is impaired. Increasing phosphate intake could worsen mineral and bone disorders, phosphate intake is usually restricted rather than increased.

Peripheral edema: Peripheral edema indicates fluid retention, which is a common complication in clients with kidney disease. While it requires monitoring and potential adjustment of dialysis, it is not immediately life-threatening compared with signs of peritonitis.

Proteinuria: Protein in the urine may indicate ongoing kidney damage but does not require urgent intervention in the acute setting. It should be monitored as part of long-term management but is not an emergency.

Anorexia: Anorexia is a nonspecific symptom that may reflect chronic illness, electrolyte imbalance, or uremia. While it warrants assessment and supportive care, it does not require immediate action in the same way that a rigid abdomen does.

Respiratory alkalosis: client with inadequate chest expansion due to muscle weakness: Inadequate chest expansion leads to hypoventilation, which causes carbon dioxide retention and respiratory acidosis, not alkalosis. Respiratory alkalosis is caused by hyperventilation, resulting in excessive CO₂ loss.

Metabolic alkalosis: client who is mechanically ventilated: Mechanical ventilation primarily affects respiratory function and gas exchange. While ventilation settings can influence CO₂ levels, they do not directly cause metabolic alkalosis, which is usually due to excessive loss of acids (e.g., vomiting) or gain of bicarbonate.

Respiratory acidosis: client with hyperventilation due to anxiety: Hyperventilation from anxiety causes excessive CO₂ exhalation, leading to respiratory alkalosis, not acidosis. Respiratory acidosis occurs when CO₂ accumulates, often from hypoventilation or impaired gas exchange.

Potassium level 5.2 mEq/L: This potassium level is slightly above normal (3.5–5.0 mEq/L) but does not usually require immediate intervention unless trending upward or accompanied by ECG changes. CRRT can help gradually remove excess potassium, making this a lower-priority concern.

Sodium level 135 mEq/L: This sodium level is at the lower end of normal (135–145 mEq/L) and does not typically require urgent intervention unless the client shows neurological symptoms or rapid shifts occur. Monitoring and gradual correction are usually sufficient.

Pulse rate 90 beats/min: A pulse of 90 beats/min is within normal limits (60–100 bpm) and does not require immediate action. It should be documented and monitored as part of routine assessment, but it is not an urgent concern in this scenario.

Hang the drain bag on an IV pole prior to infusing dialysate: The dialysate drain bag should be positioned lower than the client’s abdomen to allow for passive drainage by gravity, not on an IV pole above the client, which could interfere with proper outflow and increase the risk of complications.

Maintain the client in a left lateral position during exchanges: Positioning during peritoneal dialysis is usually supine or semi-Fowler’s to facilitate inflow and outflow of dialysate. Lateral positioning is not standard practice and may hinder proper fluid distribution within the peritoneal cavity.

Monitor vital signs every two hours during the procedure: Frequent vital signs monitoring is important for overall assessment but continuous monitoring is not typically required for stable clients during a standard peritoneal dialysis exchange unless complications such as hypotension or infection are suspected.

Client with pruritus: Pruritus is a common symptom in CKD due to uremic toxin accumulation and dry skin. While uncomfortable and important to address, it is not immediately life-threatening and does not indicate acute deterioration of kidney or cardiovascular function.

Client with a blood pressure of 158/90 mm Hg: Elevated blood pressure is common in CKD due to fluid retention and RAAS activation. Although it requires monitoring and possible intervention, this level is not immediately critical unless accompanied by symptoms of hypertensive emergency.

Client with oliguria: Oliguria indicates reduced urine output and worsening kidney function, but it is typically a gradual change in CKD and not immediately life-threatening unless accompanied by electrolyte imbalances or signs of fluid overload.
