Stridor: This intense, monophonic sound originates from the upper airway due to laryngeal or tracheal obstruction. It occurs primarily during inspiration and signals life-threatening compromise. Unlike lower airway sounds, it indicates extrathoracic or glottic narrowing rather than distal bronchiolar constriction.

Atalectic crackles: These are non-pathological sounds heard in the lung bases that disappear after deep inspiration. They result from the sudden expansion of collapsed alveoli in sedentary patients. These sounds are brief and lack the musical, continuous quality found in obstructive pathology.

Crackles- course: These discontinuous, bubbling sounds occur when air collides with secretions in the trachea and large bronchi. They are low-pitched and change with coughing or suctioning. They lack the whistling nature of wheezes and indicate excessive mucus or fluid within the airways.

Anisocoria: This term describes an inequality in pupillary diameter, which may be physiological or pathological. It results from imbalances in the autonomic supply to the iris muscles. While often seen with eyelid issues, it specifically refers to the pupil, not the lid.

Strabismus: This condition involves a lack of ocular alignment where the visual axes do not converge on the same point. It results from extraocular muscle imbalance or refractive errors. While it affects eye appearance, it does not involve the position of the eyelid.

Stye: Also known as a hordeolum, this is an acute focal infection of the sebaceous glands of the eyelid. It presents as a painful, erythematous nodule near the lid margin. It causes swelling but is not a primary neurogenic drooping of the lid.

Vertigo: This symptom is characterized by a sensation of spinning or rotational motion caused by vestibular system dysfunction. It involves the semicircular canals or central vestibular pathways. While it may co-occur with auditory issues, it describes balance, not a buzzing sound.

Pruritis: This refers to an unpleasant sensation that provokes the urge to scratch the skin or mucosa. In the ear, it often indicates otitis externa or eczematous changes in the canal. It is a tactile sensation rather than a phantom auditory perception.

Cholesteatoma: This is an abnormal, non-cancerous skin growth that develops in the middle ear behind the eardrum. It can cause hearing loss and erosion of the ossicles through destructive enzymatic activity. It is a structural lesion rather than a specific subjective sound symptom.

"I am having a hard time cleaning my dentures at night.": Difficulties with denture hygiene usually relate to impaired manual dexterity or visual deficits in the geriatric population. While dry mouth can affect denture fit, cleaning challenges are not a direct subjective report of oral dryness.

"I need to add more salt to my food than I used to.": This statement indicates age-related decline in gustatory sensitivity or potential zinc deficiency. While saliva helps dissolve tastants, the need for more salt is primarily a symptom of dysgeusia rather than a direct complaint of xerostomia.

"I can't eat as many raw vegetables as I would like.": Restricted intake of raw vegetables in the elderly often stems from masticatory dysfunction or poor dentition. While lack of saliva complicates the chewing process, this statement is more indicative of mechanical dental issues than isolated dry mouth.

Rhonchi: These are low-pitched, sonorous adventitious sounds caused by secretions in the large airways. They often resemble snoring and suggest conditions like bronchitis. They are always considered abnormal findings during a physical assessment and indicate airway obstruction or fluid.

Wheezes: These high-pitched, musical sounds are adventitious and occur when air passes through narrowed or constricted small airways. They are common in reactive airway diseases like asthma. Their presence signals a pathological state rather than an expected, healthy respiratory finding.

Crackles: These discontinuous, popping sounds are adventitious and indicate the sudden opening of small airways or fluid in the alveoli. They are associated with heart failure or pneumonia. Finding these sounds during assessment suggests an underlying respiratory or cardiac abnormality.

Tonsils are (barely) visible.: This description corresponds to a Grade 1+ classification where the lymphoid tissue occupies less than 25% of the oropharyngeal space. It is a common finding in healthy adults without active inflammation or chronic hypertrophy. This grade indicates minimal protrusion beyond the anterior pillars.

Tonsils are halfway between the tonsillar pillars and uvula.: This clinical observation defines Grade 2+ tonsils, occupying between 25% and 50% of the lateral-to-medial distance. While more prominent, they do not yet impinge significantly on the central airway. This stage is frequently seen in pediatric populations or during mild pharyngitis.

Tonsils are touching each other.: This extreme finding is classified as Grade 4+, often referred to as kissing tonsils. It represents total or near-total oropharyngeal obstruction, significantly increasing the risk of obstructive sleep apnea or respiratory distress. This stage exceeds the requirements for a 3+ grading.

Slight amount of lid lag when moving the eyes: Lid lag occurs when the superior eyelid does not follow the downward movement of the globe, exposing a strip of sclera. This is a characteristic clinical sign of hyperthyroidism or Graves disease. In a healthy individual, the eyelid should overlap the superior limbus throughout the movement.

Nystagmus in downward gaze: Nystagmus is an involuntary, rhythmic oscillation of the eyes that is generally pathological when occurring in most directions. While a few beats of horizontal nystagmus at extreme lateral gaze can be normal, nystagmus during downward gaze is an abnormal finding. It often indicates vestibular or cerebellar dysfunction.

Convergence of the eyes: Convergence is the simultaneous inward movement of both eyes toward the midline to maintain single binocular vision when viewing a near object. While a normal reflex, it is not the focus of the diagnostic positions test. This specific test is designed to assess version and muscle balance.

Epistaxis: This term refers to acute hemorrhage from the nostril, nasal cavity, or nasopharynx, usually due to vascular rupture in Kiesselbach's plexus. While hypertensive patients are at risk for both stroke and nosebleeds, epistaxis is not a direct result of the facial nerve palsy seen in stroke.

Xerostomia: Dry mouth results from decreased salivary production, often due to medications, dehydration, or Sjögren syndrome. While a stroke can affect the autonomic innervation of salivary glands, it is not the most immediate or common assessment finding associated with visible facial drooping and motor weakness.

Rhinorrhea: This is the free discharge of thin nasal mucus, commonly associated with allergic rhinitis or viral infections. It is an inflammatory or infectious response of the nasal mucosa. It does not have a direct pathophysiological link to the motor deficits or facial droop following an ischemic or hemorrhagic stroke.
