A patient requiring discharge teaching about coronary artery stenting prior to going home today: Discharge teaching for coronary interventions requires specialized knowledge of antiplatelet therapy, lifestyle modifications, and potential complications. A floating RN may not be fully equipped to provide in-depth education or answer detailed cardiac-specific questions.

A patient just transferred from the radiology department after a coronary angioplasty: This patient is considered high acuity due to potential post-procedural complications such as bleeding at the access site, arrhythmias, or acute vessel closure. Assigning this patient to a floated RN unfamiliar with post-PCI monitoring would pose safety risks.

A patient just admitted with unstable angina who has orders for a heparin infusion and aspirin: This patient is unstable and at high risk for acute coronary events. Heparin infusion requires careful titration, frequent lab monitoring (aPTT or anti-Xa), and rapid recognition of bleeding or ischemic complications.

"The medication can take up to 15 minutes to take effect.": Sublingual nitroglycerin works much faster than 15 minutes, usually relieving chest pain within a few minutes. Providing an inaccurate timeframe may delay appropriate interventions if the angina persists or worsens, potentially putting the client at risk for myocardial ischemia.

"Stop taking the medication and notify your provider if you develop a headache.": Headaches are a common side effect of nitroglycerin due to vasodilation and are generally not dangerous. Clients should be instructed that headaches can occur but do not require stopping the medication unless severe or persistent, and over-the-counter analgesics are often effective.

"Avoid taking the medication prior to exercising.": In fact, clients are often advised to take sublingual nitroglycerin prophylactically before activities that may provoke angina, such as exercise, to prevent ischemic episodes. Avoiding it before activity could unnecessarily increase the risk of angina during exertion.

Obtain a blood sample: While laboratory tests may help identify underlying causes of ventricular tachycardia (V-Tach), obtaining a blood sample is not an immediate priority in a patient who is hemodynamically unstable. The patient’s low blood pressure, altered mental status, and signs of poor perfusion require urgent intervention to restore cardiac output.

Defibrillation: Defibrillation is reserved for pulseless V-Tach or ventricular fibrillation. In this scenario, the patient still has a pulse, so immediate defibrillation is not appropriate. Using defibrillation on a pulsed patient could be harmful, whereas synchronized cardioversion is specifically indicated for unstable but pulsing arrhythmias.

CPR: Cardiopulmonary resuscitation is indicated when a patient has no pulse and is in cardiac arrest. Since this patient is still pulsing, initiating CPR would not address the underlying arrhythmia effectively and could delay definitive treatment, which in this case is synchronized cardioversion.

Blood pressure 104/56 mm Hg: This blood pressure is slightly below the normal range but may be acceptable post-PCI, depending on the patient’s baseline and clinical stability. Mild hypotension should be monitored, but it does not typically require immediate intervention unless accompanied by symptoms of shock or organ hypoperfusion.

Pedal pulses 2+: Palpable, strong pedal pulses indicate adequate peripheral perfusion. This assessment finding is reassuring and does not necessitate immediate intervention, though ongoing monitoring of circulation distal to the access site is important.

Heart rate 100 bpm: A heart rate of 100 bpm is at the upper limit of normal and may reflect mild post-procedural stress, anxiety, or pain. While it requires monitoring, it is not an immediate threat to the patient’s stability in comparison to severe chest pain post-PCI.

Lidocaine: Lidocaine is a class IB antiarrhythmic used primarily for ventricular arrhythmias. While it can cause minor changes in conduction or suppress ectopic foci, it is unlikely to cause brief runs of asystole. Its mechanism stabilizes the ventricular myocardium rather than inducing transient pauses.

Atropine: Atropine is an anticholinergic agent used to treat symptomatic bradycardia by increasing heart rate through vagal inhibition. It does not cause asystolic episodes; rather, it accelerates sinus node firing, making it an unlikely cause of transient asystole.

Epinephrine: Epinephrine is a sympathomimetic that increases heart rate, contractility, and conduction. It is used in cardiac arrest and severe bradycardia, but it does not typically produce short runs of asystole; its effect is stimulatory, not suppressive, on cardiac conduction.

E. Amiodarone: Amiodarone is a broad-spectrum antiarrhythmic that slows conduction and prolongs repolarization. While it can cause bradycardia or AV block in some cases, brief runs of asystole lasting 3–6 seconds are not a common or expected effect of this drug.

Complete blood count: While a complete blood count (CBC) provides information about red and white blood cells and platelets, statin therapy is not typically associated with hematologic toxicity. Routine CBC monitoring is not required for patients on statins unless there are specific clinical concerns or comorbid conditions.

Cardiac enzymes: Cardiac enzymes such as troponin and CK-MB are used to detect myocardial injury. Statins do not usually cause acute cardiac injury, so monitoring these enzymes is not part of routine statin therapy. However, CK (creatine kinase) may be monitored if myopathy or muscle pain develops.

Blood urea nitrogen: Blood urea nitrogen (BUN) assesses renal function. Statins are metabolized primarily by the liver, not the kidneys, and do not typically affect BUN. Monitoring BUN is not routinely required unless the patient has preexisting kidney disease or is taking nephrotoxic medications concurrently.
